JUSTIFICATION WITH ASSESMENT ; Patient Humid Mubarak come to clinic with limping pattern
and difficulty for a walk as concomitant condition due to brain stork in April 2022.On Physical
assessment of lower limb Bilateral ankle pain lt,Rt side due to chronic lateral ligament sprain .Bilateral
metatarsal on Lift and right side of foot
Anticipated Management Plane:
1) Patient is Requested for Customized insoles which will include Metatarsal Bar for reduction
of Metatarsal pain
2) 1cm of shorting was found during his gait on his right side leg thus, to over come with it he is
need of Compensation
3) On left side of foot hallux valgus of big too was found for that correction he need toe
separator
4) To Accommodate Customized insoles and compensation required high top sup/pro medical
shoes for the patient better ADLS .
 over night and also dynamic kindly approve the above mention HCPCS for patient Limbs Correction
and stability.
