Mental Health Status Examination

General Appearance and Behavior

Patient is a 6-year-old female who lives at home with her parents. The child is sitting quietly, dressed
suitably and neatly. The patient appear to be thin and dully. According to her mother, her child was born
with a physical impairment that causes her child to be unable to walk correctly due to bone weakness
and a lack of muscular coordination, causing the child to develop slowly. The patient also has difficulty
formulating words, which makes her unable to communicate correctly and express thoughts verbally.
We discovered that he bites her both hands when she suffers toothaches, creating her hands the
appearance of wounds. According to her mother, her child dislikes crowded areas and being surrounded
by others. She requires assistance in eating, bathing and even playing. She appears unhealthy due to her
malnutrition and physical disability.

Speech

The patient have difficulty in forming words and sentences. Sometimes she would just stare at you for a
minute and then she would cry loudly. This would be a big problem of the patient which she could not
express her thoughts verbally.

Mood and Affect

Patient have irritable mood to people especially if those people doesn’t she always saw. If she was given
something that she would like she would greatly accept and appreciate it with a blunted affect face. A
minute after she would cry and annoyance to the people surrounded by her.

Thought

The patient would recognize people if she was comfortable to that person. Her mother reported that
every time her child playing toys she observed that her child would speak and talk to that toy acting like
someone was playing with her. Living like her own world, she would also like dancing if she hears some
music by their neighborhood.
