Health Card - If your address change is successful, continue to use your existing health card. You will not receive a new health card
as a result of an address change.
Driver's Licence - If your address change is successful, you will receive a new driver’s licence in the mail within 4-6 weeks. If you do
not receive your new driver’s licence in the mail after 6 weeks, please visit a ServiceOntario centre or call ServiceOntario at 416-235-
2999 (GTA) or toll-free at 1-800-387-3445 (Canada).
Note : If your driver’s licence is due for renewal within the next 90 days, you will not receive a new driver’s licence in the mail.
Vehicle Permit - You need to visit a ServiceOntario centre to pick up your new vehicle permit. You will need to bring your original
vehicle permit (green ownership document) to a ServiceOntario centre and exchange it for your new vehicle permit.

If your address change was unsuccessful, you will need to visit a ServiceOntario centre to update your address.

Standards for Customer Service, March 30, 2015, under Accessibility for Ontarians with Disabilities Act, 2005 (AODA) requires that
ministries make alternate format versions of their publications available to persons with disabilities. The most frequently requested
alternate formats are Braille, large print, audiocassettes, or CD ROMs, and electronic text on diskette. For all alternate format requests
