Subject: Fire in the Breakroom on October 5, 2021
The following is an event report on an accident that took place on October 5, 2021, that resulted
in a microwave fire in the breakroom.

Accident summary
There was a fire incident in the common room for refreshments at ABC Software's office block
on October 5, 2021 Tuesday. Bob Smith, who worked in the Accounting department, was
cooking a bag of generic brand microwave popcorn when it exploded. He thought to have set the
microwave timer at two minutes, although the timer had fourteen minutes remaining by the time
of the walk-in. I put the fire extinguisher on the wall subsequent to the microwave and put out
the fire. Smoke spreads throughout the structure.
Accident details
1. Bob Smith of the Accounting Division puts popcorn in the microwave and goes outside
to smoke, thinking he set a 2-minute timer.
2. The timer does not go off after 2 minutes, which results in popcorn overheating and a fire
burst.
3. I take a fire extinguisher on the wall and put out the flames.
4. Smoke spreads throughout the building with a choking smell.
5. There are burnt remains in the microwave.
6. Firefighters are phoned at 2:38 p.m. and arrive at 2:43 p.m. from the fire station, a block
away.
7. Alan Martin (Programming Dept.) and Dennis Alcott (Programming Dept.) report
inhaling complications.
8. They are taken to General Hospital by an ambulance.

Recommendations
To prevent such accidents in the future, the ABC management should;

• Employ kitchen staff to handle any cooking professionally.


• Enhance technology by establishing fire sensors and alarms.
• Educate staff on safety measures when handling electric gadgets.
