CONSENT LETTER TO APPLY FOR A BUILDING PERMIT

KNOW ALL MEN BY THESE PRESENTS:

We, Anne A. Arquitola, widow, Peter A. Arquitola, married, and Jose Roy A.
Arquitola, married, all of the legal ages, Filipinos, and all residents of Baroro, Bacnotan, La
Union, co-owners of property in Baroro, Bacnotan, La Union, covered by Transfer Certificate of
Title No. 025-2020000696, Tax Declaration Number 2020-03-0008-00019, designated as Lot 3-
B under Approved Subdivision Survey Plan Number PSD-01-099081 and Transfer Certificate of
Title No. 025-2020000697, 2020-03-0008-00020, designated as lot Lot 3-C, and in the same
Approved Subdivision Survey Plan Number PSD-01-099081, hereby give our consent and
authorization to Paul A. Arquitola, married, of legal age, Filipino citizen, and resident of Baroro,
Bacnotan, La Union, to apply for and secure a building permit from the appropriate government
agency, on our behalf, for the construction of a building on our property we own located at
Baroro, Bacnotan, La Union.
We hereby certify that we are the lawful owners of the said property and that we have
given the applicant our full consent and authorization to apply for and secure the necessary
building permit for the construction of the building on our behalf. We understand that this
consent and authorization shall remain valid until the building permit is issued and the
construction of the building is completed.
We hereby waive our right to question the authority of Paul A. Arquitola in applying for
and securing the building permit on our behalf. We also agree to hold Paul A. Arquitola free and
harmless from any and all liabilities, claims, and demands arising from the application and
securing of the building permit.
