1. That my maiden name is Ms. Anjali Ramakant Singh
2. That I got married to Mr. Arvind Rambachan Singh on 18th March, 2023 at
Chandivali (Meera Banquette) vide marriage certificate having number
098877E, issued by the Department of Registration & Stamps- Government
of Maharashtra.
3. After marriage my name is Ms. Anjali Arvind Singh Sushant
4. That in my marriage certificate both the names mine and the name of my
husband are stated.
5. That my Aadhar number is 0909 9090 7879 which is the same as before and
after marriage.
6. That my Pan number is BXPCY28U which is the same as before and after
marriage.
7. That the said expanded initial namely Anjali, may kindly be treated as the
first name, Arvind may be treated as the middle name and Singh as the last
one.
8. I State that Ms. Anjali Ramakant Singh and the present Ms. Anjali Arvind
Singh is the name of the same person and that is myself.
I am executing this declaration to be submitted to the concerned authorities for
the change in my name.

known all to whom these Present shall come that I/we Mr. Thatikayala
Chukkaiah The above-named do hereby appoint Mr. Prashant Choudhary
(hereinafter called the advocate/s) to be my / our Advocate in the above–noted
case authorize him: -
To act, appear and plead in the above-noted case in this court or in any
other court in which the same may be tried or heard and also in the appellate
court including the High court subject to payment of fees separately for each
court by me/us.
To sign, file, verify, and present pleadings, appeals cross objection, or
petitions for execution review, revision, withdrawal, compromise, or other
petitions or affidavits or other documents as may be deemed necessary or proper
for the prosecution of the said case in all its stages subjects to payment of fees
for each stage.
To file and take back documents, to admit and/or deny the documents of
the opposite party.
To withdraw or compromise the said case or submit to arbitration any
differences of disputes that may arise touching or in any manner relating to the

To take execution proceedings on paying a separate fee.
To deposit, draw and receive money, cheques, cash, and grant receipts hereof
and to do all other acts and things which may be necessary to be done for the
progress and in the course of the prosecution on the said case.
To appoint and instruct any other Legal Practitioner authorizing him to exercise
the power and authority hereby conferred upon the Advocate whenever he may
think fit to do so and to sign the power of attorney on our behalf.
And I/we undersigned do hereby agree to ratify and confirm all acts done by the
Advocate or his substitute in the matter as my/our acts, as if done by me/us to
all intents and purposes.
And I/we undertake that I/we or my/our duly authorized agent would appear in
court on all hearings and will inform that Advocate for appearance when the
case is called.
And I/we undersigned do hereby agree not to hold the advocate or his substitute
responsible for the result of the said case. The adjournment costs whenever
ordered by the court shall be of the Advocate which he shall receive and retain
for himself.
And I/we undersigned do hereby agree that in the event of the whole or part of
the fee agreed by me/us to be paid to the advocate remaining unpaid, he shall be
entitled to withdraw from the prosecution of the said case until the same is paid
up. The fee settlement is only for the above case and the above Court. I/We
hereby agree that once the fee is paid, I/We will not be entitled to the refund of
the same in any case whatsoever, and if the case prolongs for more than 3 years
the original fee shall be paid again by me/us.
2. I have not made any Will or other Testamentary document, but if any are
made, I hereby revoke all previous Wills and Codicils, if any, and declare this is
to be my last Will and Testament.
3. I appoint Mr. Ameya Pramod Ghagare as the Executor of this Will and
trustee of my estate. In the event, he predeceases me, then only in such a
situation, do I wish to appoint Mr. Danesh Zehen, as, my executor.
4. During my lifetime and with my own physical and mental efforts, ability, and
capabilities, I have acquired the below-mentioned immovable properties which
are as under: -
a. 100% right, title, and interest in shop no. B/11, Sheetal Radeon, Ground
Floor, Topiwala Nagar, Khar(E), Mumbai – 400098.
b. 50% right, title and interest in B/3, Sheetal Radeon, 1st Floor, Topiwala
Nagar, Khar (E), Mumbai – 400098.
7. In the event of my wife surviving me, I hereby declare that my wife, Sudha
Aditya Singh, shall have a life interest in all my assets, movable and
immovable. The entire income generated from my assets shall be provided to
my wife during her lifetime for her needs and necessities. The corpus of the
investments, during her lifetime, shall be utilized for her medical expenses, if
the need so arises. The bequeath of the estate shall only be effective after the
heavenly abode of my wife, Sudha Aditya Singh.
10. In respect of the other movables, I Bequeath all my fixed deposits and Cash
to ), Pankti Kantilal Prasad (Granddaughter), Yashvi Kantilal Prasad
(Granddaughter), Nilay Eklavya Singh (Grandson), Stuti Eklavya Singh
( Granddaughter), Labhya Eklavya Singh (Grandson) in equal ratio.
11. If any of the assets that have remained to be listed in this Will, the same
shall stand Bequeathed to Adeeb Ul Furkan Zia-ul-Haqh Sheikh.
12. Needless to mention that before the distribution of the monies parked in the
saving accounts and the fixed deposits, my funeral expenses shall be deducted
and paid over to the person who has carried out the said expenses. Similarly, all
the expenses for the maintenance of my properties and/or any other liabilities
that are required to be discharged after the heavenly abode shall be carried out
by the Executor appointed under this Will. All such expenses for maintenance
as well as discharging of liabilities including all legal fees, court fees, and/or
any other incidental expenses that are carried out for probating and executing
my last wishes as narrated in this Will, shall be first reimbursed to the Executor
from a savings account as well as fixed deposits and only the remainder thereof
shall be distributed in terms of the clauses of this Will.

To All To Whom These Presents Shall Come. I Mr. Aditya Arvind Singh,
aged about 50 years, Indian Inhabitant, having a present address at Flat no 69,
Hera Pehri Apartment, Gokul Village, Madrasan Park, Malad (E), Mumbai
400088, do hereby SEND GREETINGS.

WHEREAS, I above/myself the legal owner of Shop No. 34, admeasuring
200sqft Built-up/carpet i.e. equivalent to 2-99sq.meter, built up/carpet, on
the ground floor C/D Wing, Type B/A1, building known as “PAVILION
CLUSTER 1”, Society known as Pavilion Estate Cooperative Housing
Society Ltd., Gokul village, Madrasan Park, Malad (E), Mumbai-400088,
owned by me and have actual possession of the said shop and all this day is the
occupation of the said property.
AND WHEREAS on account of my poor health and as I have been advised to
take bed rest, therefore I am personally unable to look after and manage all the
affairs of the said shop property and therefore I desire to appoint a fit and proper
person as my true and lawful attorney to look after and manage all the affairs of
the said property in the following manner.

1. To file a complaint, application, case, suit, affidavit, and all necessary
documents and to appear before all concerned government authorities, offices,
and court to file suit, case, application, affidavit, and all necessary documents
and to represent me/us before the Hon’ble Court, Thane, Mumbai, Maharashtra
State, and India to appear and defend of me/us till the finalization/disposed of
any case/suit and appear before competent authority or any other incorporated
body of association concerning the said property.
2. To settle and compromise any claims, disputes, suits, litigations, and deal in
all the litigation, disputes, and monetary transaction to be settled with in respect
of the said Shop Property with all or any government and local authorities,
court, revenue authority, cooperative court, collector and more particularly of
the said property.

5. To declare, and affirm all plaints, written statements, applications, petitions,
affidavits, undertakings, and other necessary documents in my/our name and on
my/our behalf and to appoint my/our advocate, solicitor, or other legal experts
to plead and appear on my/our behalf before any Court of Law, Civil Criminal,
Revenue Municipal, Cooperative courts, and other tribunals, constituted by any
act rules from time to time and appear before revenue Authority registrar/Sub
Registrar of Assurance, Co-operative Registrar, Municipal, Income Tax
Authorities and or any other Officer or Officers empowered by law to hear any
such proceedings or inquires pertaining to any other matter of the said property.
6. To accept any notice or services or writ or summons or other legal proper that
may be several upon me/us pertaining to the said property.
7. To pay all taxes, assessments, rent, charges, and bills, pertaining to said
property.
9. This power is Irrevocable and shall not be revoked by me/us and will be
binding on my/our heirs, executors, administrators, and assigns.
10. To substitute and appoint from time to time one or more ATTORNEY with
the same or limited powers and substitute or substitutes at pleasure to remove
and to appoint another or others.

AND GENERALLY, to do all such acts, deeds, and things that my/our said
Attorney may deem fit and proper as the nature and circumstances may deem
necessary from time to time in respect of anything connected with the said
property, and I/We do hereby agree and undertake to ratify and confirm all such

medical check-ups and visiting Doctors and Hospitals in order to get the patient
