DISCUSSION

 DUTY DETAILED AND ADDITIONAL ASSIGNMENTS:

 CTSO1 ROSELLE COLANTA will be the TB DOTS nurse to be train

in preparation for the retirement of CTSO4 GLORIA VILLAMOR.
 CO2 JOPHET CRUZ will be the admin staff which includes the
workload in making the attendance and reports for monthly,
quarterly and annually. Also in addition, CO2 JOPHET CRUZ will
render his duty at LRP Infirmary for security purposes as the need
arises.
 CTSO1 ANGELINA SEVILLE will in-charge in making the purchase
request and should keep an original file copy.
 CTSO4 GLORIA VILLAMOR is the one in-charge in making the
schedule of duty detail.

 All staff who will change their duty & OFF duty should make a
letter request signed by the concerned, supervisor & Chief, HSD
before/ after the said date and give a copy to the time keeper. The
Chief, HSD & supervisor should also be informed ahead of time in
case of emergency.
 Nurse on Duty (NOD) are all reminded & required to make charting
as it is one of your work as a nurse and to monitor the case of the
patient. Make a call to the Physician if it is an emergency and need
to report the diagnosis immediately.
 NOD especially the afternoon and night shift should know the
paper works especially the ISO FORMS USED and also they
should encode and update the Inmate Medical Records (IMR)
regularly. Don’t leave all the paper works to the ADMIN staff so
that all the staff should know the workflow of the paper works.

 DEATH OF PDL:

 NOD should report immediately to the physician for the diagnosis.

At the same time produce a notice of death for the report and
prepare the other documents if the PDL died at night or early
morning don’t leave your post when not in properly endorsed to
the next shift.
 Tap the overseer section for the Time, in claiming the cadaver
that it should be office hour for the reason of the availability of the
signatories.

 SCHEDULE OF CONSULTATION:

 Once a week for the medium and minimum security compound in

an alternate basis starting second half of the month of January
2023.

 OFFICE AT MEDIUM SECURITY COMPOUND:

 CTSSUPT SUSAN URGEL will tap the GSS and make a request for
the repainting and making of cabinet, medicine steel & etc. And also
a request for additional nurses that will be assigned at Medium/
Minimum Security Compound.

 Relocation of the Barber shop at Maximum Security Compound for

security purposes should be tackled on staff meeting (Chief of
Section/ Division).
