እኔ ስሜና አድራሸዬ ከላይ የተገለፀው የሞባይል ቁጥር _______________________ ተጠቃሚ የሆንኩ፣ በዚህ ሞባይል ቁጥር ከአንድ ዓመት በላይ
የተገለገልኩበት መሆኔን አያረጋገጥኩ፣ ደንበኝነቱ በስሜ እንዲዘዋወርልኝ እጠይቃለሁ፡፡ የሞባይል ቁጥሩ የድህረ ክፍያ ወይም ሃይብሪድ ከሆነ፣
ከላይ በተጠቀሰው የሞባይል ቁጥር የስምና ባለቤትነት ዝውውር እስከተፈጸመበት ዕለት ድረስ የቆጠረውን የቢል ክፍያ ለመክፈል ኃላፊነቱን
የምወስድ መሆኔን በፊርማዬ አረጋግጣለሁ፡፡

I confirm with my signature to return the mobile number if the prior customer request the mobile number within a year
and to be responsible if any of the information given above found to be false or for any violations against the law while
using the mobile number. I also confirm to take responsibility to pay any outstanding payments that might be incurred
before the return date to the prior customer per his/her claim.

የሞባይል ቁጥሩ በስሜ ከተዘዋወረበት ቀን አንስቶ በአንድ ዓመት ጊዜ ውስጥ የቀድሞው ባለቤት የሞባይል ቁጥሩን መልሶ ለመጠቀም ጥያቄ
ካቀረበ እንዲሁም የሰጠሁት መረጃ ትክክል ሆኖ ባለመገኘቱ ምክንያት ወይም ቁጥሩን ከህግ አግባብ ውጪ በመጠቀም ለሚፈጠረው ማንኛውም
ችግር በኃላፊነት የምጠየቅ መሆኔን በፊርማዬ አረጋግጣለሁ፡፡ እንዲሁም ሞባይል ቁጥሩ ለቀድሞ ደንበኛ ከመመለሱ በፊት የቆጠረውን
የአገልግሎት ክፍያ ኃላፊነት ወስጄ ለመክፈል የተስማማሁ መሆኔን በፊርማዬ አረጋግጣለሁ፡፡
