Incident Learnings
Location: Azimarine Fabrication Yard, QIT, Ibeno LGA
What Happened
September 12, 2018: A personnel hit his right sheen on the flange end of a small pipe spool as he attempted to hook-up the load
(pipe spool) with a tag line (for crane lifting), having slipped off a nearby spool which he climbed on to access the previous spool,
requiring First Aid Treatment.
How bad was it?
Minor Abrasion (took hours – days to heal)
How Bad Could It Have Been?
Major Leg Dislocation
What Lessons were Learned?
▪ Inadequate hazards identification & mitigation can cause incidents. The hazards associated with walking on the pipe
spool (which had slippery surfaces) were not identified and as such were not mitigated.
▪ High Risk Tolerance/Complacency often results in incidents. History of performing the task in a similar way (personal
experience/familiarity with task) with known hazards did not result in any negative consequences or hurt. When complacency
sets in, there is bound to incidents.
▪ Poor intervention. Co-workers or task lead did not intervene to stop unsafe condition (slipping hazard) or stop IP from
working in an unsafe area. Poor supervision and lack of team work will usually result in incidents.
Corrective Actions Taken
▪ Last minute risk assessments (LMRAs) to be conducted prior to commencement of any task, to identify & mitigate hazards.
▪ Adequate supervision, mentoring and coaching (especially for new workers) will always be provided for all tasks to ensure
any possible work limitations are addressed.
▪ All personnel are encouraged to ensure a safe work environment & encourage U-See U-Act; take actions to correct an
unsafe condition.
▪ Personnel are encouraged to show their co-worker that they really CARE. They must intervene to stop an unsafe act.
▪ All personnel must care for their co-worker. They must intervene to correct an unsafe act & stop an incident.
▪ Personnel must apply Stop, Think & Act for all tasks – if site conditions change; work must be stopped and re-assessed.

Significant Near Miss Learnings
Incident: Near-miss Location: Azimarine Fabrication Yard, QIT, Ibeno LGA.
What Happened
August 18, 2018. A small pipe spool dropped on a low-bed truck almost rolled over from the truck, owing to non-placement of a
wedge immediately.
Immediate response
The rigger and the truck driver immediately rushed to place a wedge to support the load, to prevent it from falling off the truck.
What lessons were learned?
▪ Whenever a load is in transit, the supporting wedge for its placement must be placed close to dropping area on the truck
(and placed immediately the load is dropped, to secure the load) so as to prevent roll over of the load.
▪ Even when the load is secure on the truck, unauthorized persons are not expected anywhere near the truck, to avoid
crushing by the load in case of any disconnect of the wedge. Thereafter, a sling rope should be used to wrap around he load
to the truck to ensure it is firmly secure.
▪ Adequate and visible caution signs/barriers must be placed / traffic man stationed to alert nearby personnel of possible
dangers e.g. dropped objects around the work site.
Actions taken to prevent reoccurrence
▪ A near miss training session was organized for all the personnel to discuss what actually happened, what could have gone
wrong, and ways to ensure such near misses do not happen again.
▪ Daily tool box talks are now held everyday, from where hazards associated with each tasks are discussed as well as ways to
mitigate such hazards.
▪ Periodic/regular site inspections are now carried out by site lead to ensure work conditions are always safe for every task.
▪ Zero tolerance to risks. Workers are encouraged not to carry out any task if they perceive such tasks as possessing high
risks to personnel, machinery or the environment.
▪ Adequate supervision, mentoring and coaching will be provided for all tasks, especially for new workers. This is aimed at
addressing any possible work limitations which may have the potential to affect the safe execution of such tasks.
▪ Only competent and trained supervisors and work crew are engaged to carry out lifting activities, to avoid
incidents/accidents.

Company Initiatives
The Management of Azimarine & General Services Limited initiated a Safety Champion Program. The program works
as follows:
A personnel is selected from each department on a rotatory basis to pioneer/champion all safety processes/standards
which AZIMARINE wants to implement, for a period of two weeks.
Goals
▪ Ensure effective implementation of company safety policies and standards and their sustainability.
▪ Ensure effective reporting of all unsafe acts and conditions by all personnel.
▪ Ensure workers who excel in implementation of safety standards are adequately rewarded (appreciated).
Successes
▪ Improved safety culture among the workforce, e.g. conducting house-keeping at the end of every tasks.
▪ Significant decline in the number of near misses and accidents/incidents recorded at the work site.
Challenges
▪ Inadequate reporting of unsafe acts (U-See U-Act), which in turn affects some of the intervention/corrective
measures to be put in place. We take time to enlighten the workers that reporting unsafe acts and unsafe
conditions do not lead to disciplinary actions. Rather, that they are meant for corrective actions and mitigation
purposes. We also advise them to always inform their supervisors whenever they see unsafe acts or unsafe
conditions, and cannot put such in writing.
Employees buy – in
▪ The employees buy-in on the program is massive because its implementation has created more safety awareness
among the workforce and also resulted in safer working environments.
Next steps for the program
▪ Introduction of an MD/CEO Safety award for the best personnel on safety performance (monthly and yearly), as a
way of encouraging the workforce to show more seriousness to safety issues..
▪ Issuance of badges with inscription such as “nobody gets hurt” to all personnel and pasting of the inscription on all
offices to ensure everyone is informed.

Incident Learnings

What Happened
March 19, 2019: A food vendor slipped on the gratings and fell while trying to enter the waiting room of the Fabrication Yard to
serve one of her customers, requiring First Aid Treatment.
How bad was it?
Minor Abrasion (took hours – days to heal)

What Lessons were Learned?
▪ Inadequate hazards identification & mitigation can cause incidents. The hazards associated with walking on the
gratings (with flip-flop slippers) were not identified and as such were not mitigated.
▪ High Risk Tolerance/Complacency often results in incidents. History of moving around the area in the same manner
without recording any injuries/accidents. The lady thought she was going to be lucky all the time, with putting on flip-flop
slippers to walk on the gratings and not get hurt.
▪ Poor intervention. Workers or site lead did not intervene to stop unsafe act (wrong PPE) or stop IP from walking in an
unsafe area. Poor intervention/supervision will usually result in incidents.
Corrective Actions Taken
▪ The lady was stopped from entering the waiting room to serve her customers. Rather she was advised to erect a nearby
kiosk where the workers can go to get food, whenever they are in need.
▪ All personnel are advised to always focus on the direction of movement, to ensure they observe any unsafe condition
(possible obstruction) on the walk path.
▪ The condition of the gratings was re-assessed to ensure it was still safe for use.
▪ Personnel are encouraged to show their co-worker and visitors that they really CARE. They must intervene to stop an unsafe
act. They must also wear the right PPEs at all times especially around the Fabrication Yard.
▪ Personnel must apply Stop, Think & Act for all tasks – if site conditions change; work must be stopped and re-assessed

Significant Near Miss Learnings
Incident: Near-miss Location: Azimarine Fabrication Yard, QIT, Ibeno LGA.
What Happened
December 18, 2018. A low-bed truck driver while trying to reverse his truck hit a crane which was parked nearby, causing damage to the truck
rear light as well as chest problems to the driver as he hit his chest on the steering.
Immediate response
The truck driver was immediately taken for A&D tests, and the results showed negative (-). He also received treatment for chest pains.
What lessons were learned?
▪ Whenever a truck driver is about to move forwards or backwards, he must be accompanied by a flag man, who direct and control all
traffic, to ensure the truck does not a hit any nearby person, vehicles or structure.
▪ If the truck carries a load, the driver must ensure the load does not obstruct his rear view, if he’s to move backwards.
▪ Adequate and visible caution signs/barriers must be stationed to alert nearby personnel of possible dangers e.g. dropped objects around
the work site.
▪ The truck must also be fitted with reverse alarm which will beam as the vehicle moves backwards, thereby alerting nearby personnel of
the truck movement and the accompanying dangers.
Actions taken to prevent reoccurrence
▪ A near miss training session was organized for all the personnel to discuss what actually happened, what could have gone wrong, and
ways to ensure such near misses do not happen again.
▪ No task no matter how urgent must be carried out without proper approval/authorization. Personnel who go against the rule with face
severe disciplinary actions.
▪ Daily pre-checks/inspections of all vehicles, equipment and machinery are carried out by site lead and operators/drivers to ensure
conditions are safe, before they are run.
▪ Zero tolerance to risks. Workers are encouraged not to carry out any task if they perceive such tasks as possessing high risks to
personnel, machinery or the environment.
▪ Adequate supervision, mentoring and coaching will be provided for all tasks, especially for new workers. This is aimed at addressing any
possible work limitations which may have the potential to affect the safe execution of such tasks.

At about 11:40hrs, while air-lifting operation was ongoing at the Eastern Fence Area, suddenly the air
hose from the air tank pulled out at the connection clip position. Immediately, the compressor operator
turned off the engine of the compressor.

PROBLEM STATEMENT: (Do we have a problem?)

1. The air-hose connection line is disrupted.

DIRECT CAUSES
• Bulldog clip not holding the hose firmly with the air-tank delivery outlet.

running the compressor engine.
