Interdisciplinary approach to
caring
● An interdisciplinary approach involves team
members from different disciplines working
collaboratively, with a common purpose, to set
goals, make decisions and share resources
and responsibilities.
PHARMACIST AS PART OF THE
CARE TEAM
● Pharmacists are active members of the
patient care team, responsible for
overseeing the optimal, safe and
cost-effective medication therapy
management using evidenced-based
medicine to improve overall patient care in
all patients.
How Pharmacists Improve Patient
Safety
● Pharmacists are involved in all processes of
care - including product selection, dosing,
route selection, regimen review, ensuring
safe medication administration, monitoring
for desired therapeutic outcomes,
identifying and avoiding medication errors
and adverse drug events, etc.
How Pharmacists Improve Patient

● Pharmacists review all non-emergent medications
orders prior to the first dose being administered to a
patient
● Pharmacists include appropriate indications for each
patient medication order during the order
entry/transcription process to eliminate nursing and
physician confusion regarding the use of the
medication
● Pharmacists clarify all vague medication orders with
the prescriber in an effort to facilitate safe and
effective administration of patient medications
What is the role of pharmacists in medical
practice?
● Clinical pharmacists provide integrated team-based
care. They are a part of the primary care team and
provide individual, face-to-face consultation on
chronic medical conditions such as hypertension,
diabetes, hyperlipidemia, HIV/AIDS, and others. Our
high risk, difficult to control patients are referred to
the pharmacists for a comprehensive evaluation of
their condition and barriers to achieving healthy
outcomes, and these are addressed.
Pharmacist Providing Patient Care
● Clinical pharmacists are more directly involved with the
co-management of patients with high risk conditions
and offer a different perspective on disease
management. Having the clinical pharmacist
integrated into the primary care team is a plus to the
patient as well as to the team.
● The practice has been enhanced due to the additional
professional expertise that the clinical pharmacists
bring to the care of the patients, particularly in the
areas of pharmacotherapeutics and medication
adherence.

patient counseling, and creating drug
therapy regimens are important component of health care
which are the pharmacist’s responsibility.
● The beauty of a pharmacist-led clinic is that patients can come
in and see us more frequently for monitoring and to address
any questions or concerns they have regarding their medication
regimen.

Key principles
● Healthcare professionals should adapt their consultation style to the needs of individual
patients so that all patients have the opportunity to be involved in decisions about their
medicines at the level they wish.
● Establish the most effective way of communicating with each patient and, if necessary,
consider ways of making information accessible and understandable
● Offer all patients the opportunity to be involved in making decisions about prescribed
medicines. Establish what level of involvement in decision-making the patient would
like.
● Be aware that increasing patient involvement may mean that the patient decides not to
take or to stop taking a medicine.
