Patient PRIVADO S. BORJA last October 8- 11, 2017 with a final diagnosis of ACUTE GASTRITIS

I admitted the above patient with an initial /admitting diagnosis of ACUTE GASTRITIS/ SYSTEMIC
VIRAL INFECTION (SVI) because of his chief complaints of epigastric pain, dyspnea with cough and
dizziness. I am the Resident on duty only on that day October 8, 2017, day shift, but the succeeding
rounds for the patient was done by Dr. Romeo Tapales and discharged patient by Dr. Yolanda
Tenorio who made the final diagnosis of ACUTE GASTRITIS based on her assessment to the patient. It
just happened that I admitted the patient hence I signed the Philhealth Claim as a policy of Holy
Rosary Hospital. What I did is just an initial diagnosis based on my assessment to the patient on
admission. Please refer to ANNEX A2 –Physical examination of patient under Review of system-
patient had positive heart burn hence initial diagnosis of Acute Gastritis and under the Physical
examination, patient had harsh breath sounds on both lung field during auscultation and patient had
cough hence initial diagnosis of SVI

Patient : IRENE AZUL BARLETA ADMITTED LAST 21-26 October 2017 with a final diagnosis of
ISCHEMIC HEART DISEASE (I25.9) AND URINARY TRACT INFECTION (N39.0)

I admit the above patient with a chief complaints of cough for 1 week, chest pain and headache with
difficulty of breathing (refer to annex C- history and physical examination). Patient had wheezing on
both lung field with mucus rales hence the initial diagnosis was ASTHMA to consider Pneumonia.
However the succeeding Doctors who made the rounds have different assessment on the patient
hence patient was discharged with a different diagnosis based on the doctor’s own assessment to
the patient ECG result and Urinalysis result. The patient was discharged by DR. Melba Chan.(please
refer to Doctor’s order sheet)
