AFFIDAVIT TO USE THE SURNAME OF THE FATHER

I, ALFREDO M. GARCIA, of legal age, Filipino citizen, with residence and

postal address at P-1-10 Manlunas Street, Airmen’s Village Villamor Air Base,
Pasay City, Metro Manila, after having been duly sworn to in accordance with law
hereby depose and say:

1. That I am the father of MICA ALIEYAH AGUILAR who was

registered at the Local Civil Register of Pasay City under Reg. No. 2008-2561;

2. That I am seeking that my daughter MICA ALIEYAH AGUILAR

may use my surname in her Certificate of live birth;

2. That Cherlinme B. Aguilar being the Biological Mother have agreed

that our child shall be registered using my surname;

3. That, my child MICA ALIEYAH AGUILAR, was born on March 19,

2008 at 237-11 11-16th St., Villamor Air Base, Pasay City, Metro Manila;

4. That I voluntarily acknowledged and recognized MICA ALIEYAH

as my daughter who shall be entitled to all rights under the law;

5. That, I hereby request the City Civil Registry Office of Pasay City
that my child shall be known as MICA ALIEYAH AGUILAR GARCIA;

6. That I am executing this affidavit while in full possession of my

mental faculties and made herein in accordance with the provisions of R.A. 9255.

6. That I am executing this affidavit while in full possession of my

mental faculties and made herein in accordance with the provisions of R.A. 9255.
