LETTER OF CONSENT AND AUTHORIZATION

Employment and Financial Market Checks

I hereby authorize Alliance Bank Malaysia Berhad (ABMB) to conduct the following pre-employment checks on me for
purposes of assessment and evaluation as part of the employment application with the bank. I hereby agree to the
following;

i) Employment checks with my current and/or former employers for seven years prior to the date of
this employment application and/or any future job opportunities/application. I acknowledge that
such checks would include my complete employment history and details of any disciplinary
proceeding/action irrespective of whether such disciplinary proceedings have been concluded or
initiated after I left employment.

ii) FMAM to disclose facts and details on financial market misconduct committed by me, including
disciplinary proceedings under the Asian Institute of Chartered Bankers (AICB)-FMAM Joint
Disciplinary Scheme, irrespective of whether such proceedings have been concluded as at the date
of the Bank’s written inquiry with the FMAM. (if applicable)

If no, please be informed that ABMB will only conduct the employment checks with your current employment
only after your resignation upon acceptance of the job offer with the bank.

Acknowledgement and Release

I irrevocably release any contractual binding that limit my current/former employers and where relevant, the AICB,
and/or FMAM from disclosing any information about me to the Bank and irrevocably authorize them to disclose any
information but not limited to the information stated in item (i) & (ii) above.
