Chapter I

INTRODUCTION

Rationale of the Study

Technology has made human labour faster and easier in every office today.

Reducing time and effort in accomplishing tasks along with security of records are

essential for the office to survive in the modern world. Healthcare Information and

Management System (HIMS) is a computer-based system that establishes an orderly

and systematic method of managing hospital transactions more effectively and

efficiently. The healthcare industry is in need of many revolutions and turned to be

technically advanced as per those. Healthcare Information and Management System

(HIMS) is the perfect tool now widely usable by hospitals to handle a huge range of

patients and provide high-quality treatment to them. This is a hospital-wide healthcare

data management solution designed to look after various administrative (billing, bed

availability, drug store, revenue statistics) and clinical (patient record, clinical data for

research) functions.

Dr. Rogelio Garcia Memorial Hospital was established way back in 1983 of

February. On 19 July 2021, Dr. Rogelio Garcia Memorial Hospital officially transitioned

to Adventist Medical Center Iligan (AMCI)-Tupi Branch, an Infirmary private hospital with

12 beds capacity. The hospital didn't have any existing system at that time and it is said

that all transactions are done manually from keeping records, patients data records,

drug prescription and billing records. Basically, the use of a log-book is the easiest way

1

they have that time. Moreover, the registration of patients is done by just writing the

patient's name, age, address and gender. Bills are generated by recording the price for

each service provided to the patient on a separate sheet and at last they all are

summed up. Diagnosis information to patients is generally recorded on the document,

which contains patient information. All this work is done manually by the receptionist

and other operational staff and a lot of papers need to be handled and taken care of.

The current manual system has a lot of paperwork. To maintain the records of

sale and service manually is a time-consuming task. With the increase in databases, It

will become a massive task to maintain the database. Requires large quantities of file

cabinets, which are huge and require quite a bit of space in the office, which can be

used for storing records of previous details. The retrieval of records of previously

registered patients will be a tedious task. Lack of security for the records. Anyone

disarranges the records of your system. Keeping track of all activities and their records

on paper is very cumbersome and error prone. It is also not economically and

technically feasible to maintain these records on paper. Observing the continuous

increase in population and number of people visiting the hospital. Primarily, the hospital

needs to aid the immediate retrievals of patients’ information, administrator access to

modify records of the patients, generating of reports (patient's history report, laboratory

reports, pharmacy reports and billing reports) in various registers and real-time

monitoring of beds availability. All this work is done manually by the receptionist and

other operational staff and lot of papers are needed to be handled and taken care of.

Doctors have to remember various medicines available for diagnosis and sometimes

miss better alternatives as they can't remember than at that time.

Adventist Medical Center-Iligan Tupi Branch Healthcare Information and

Management System enables the hospital to develop their organization and improve its

effectiveness and quality of work. It also enables hospitals and doctors to better serve

their patients and keep their work easier to manage. All the process is stored in the

database so that they won’t be having any problem about storing and finding the

reports. Their mission is to give the best health care to all patients. Their vision is to

become better healthcare. Their goals are to continuous improvement in visibility,

competitiveness, accessibility, total health management, communication and health

education. So, it is concluded that the system should be implemented.

Dr. Amado D. Cope Memorial Hospital Patient Information

System by Barcel et al., (2017). Barcel and Rodriguez of STI College

aimed to develop a Patient Information System of Dr. Amado D. Cope

Memorial Hospital, targeted for the automation and computerization of the

hospital transaction to promote effectiveness and efficiency in terms of its

function. They also aimed to solve the problems encountered by the

hospital. They found out that updating, searching and retrieval of records

was achieved slowly, and the billing section also used the manual

computation of medical bills or payments.

management system that will upgrade the quality of information

management and efficiency of the hospital employees using this study.

The study was able to produce the module that would administer patient

registration and admission details; a module that would manage the

admission bills and pharmaceutical payments; and a module that could

monitor medicine inventory of the hospital pharmacy.

Furthermore, the study used the Waterfall Methodology model that

followed a linear structure starting from preliminary investigation,

requirement analysis, design phase, implementation and maintenance.

Each phase guided the researchers in the development of the study and

helped them organize the workflow of each task.

In conclusion, the researchers found out that the system could

speed up the working progress and productivity of the hospital employees.

It could also generate hospital reports that could help the users to provide

an overview of the hospital transaction within a specific date. It also

provided the facility for searching the details of the inquiring patient in the

receptionist module. The system could reduce the workloads in the

hospital resulting in better management and working performance. In

general, the study resulted in a better improvement of the hospital

transactions.

It has been recommended that there was a need to enhance the

front-end design of the system; add modules for the laboratory and

PhilHealth; and improve billing modules, add functions to cater more

discounts and detailed enumeration of each receipt.

Barangay Villamonte Health Center Management System by

Pescadero et al., (2018). The study is proposed to be implemented in the

said barangay. It is designed for keeping and monitoring resident’s

medical information in Barangay Villamonte. By this, paper works and

manual record keeping will be minimized. Therefore, allowing the nurse

and staff to feel at ease in keeping track of patients and increasing the

number of them served. The implementation of the Barangay Health

Management System provides a quicker and more systematic way to

record the medical information of the patients. It also provides reliable and

secured personal medical records.

Every consultation or any transactions of the patients will be

recorded or stored into a database including the monitoring and updating

of the patient health records, how many patients accommodated daily,

sickness or complaints report and the medicines released. It will simply

increase the performance of medical personnel through its automated

information system.

The system got an overall result of “4.68” which indicates that the

users “Very Satisfied” with the usefulness of the Barangay Health

Management System. This means that the system meets the objectives of

the study. It also indicates that the system is able to provide the barangay

health center a system that performs accordingly to its functions and

purpose. In other words, it has addressed the necessary automation

requirement of the center after the thorough system study.

Hospital Information Department (HID) using PowerBuilder by

Ren et al., (2017). Hospital information system is considered as one of the

most important branches of the Medical Informatics by the International

Academia Community, with the essence of integrating all the hospital

departments into a large information network to facilitate the whole

hospital system. Given the issue of “difficult to be hospitalized” in China, a

set of safe, stable and easy-to-handle beds resource management

information system was developed by the Hospital Information

Department (HID) using PowerBuilder, the MVC model and the Oracle

database. This system improved the efficiency of bed resource

management, enabled interdisciplinary collaboration across departments,

and significantly reduced the average hospital stay of patients.

In Israel the system is fully implemented with work instruments that

provide clinical safety and cost containment. The system provides

practical patient-related needs: medical files, correspondence,

dictionaries, summary and reports, laboratory test results, prescription,

assessment scales and patient labels. The patient information system is

exceptionally user-friendly and thus readily accepted by the medical staff

at the hospital.

Al Akhawayn University Health Center Management System by

Ikhanjal et al., (2017). The objective of this capstone project is to create a

secure, easy-to-use application for the management of day-to-day

operations of the Health Center at Al Akhawayn University, which will help

provide efficient and effective quality health care. The application is called

“The AUI Health Center Management System'' and its main goals are to

eradicate the use of pen paper from the Health Center and automate the

administrative processes. There is nothing appealing about paper

documentation – especially filing, shuffling and stacking – and handwritten

paper easily fades. Thus, an electronic management system safeguard

against the misplacing of patient records, reduces retrieval time, enhances

organization and, most importantly, ensures accountability as every

keystroke is documented. The time-consuming nature of maintaining

handwritten records makes a visit to the Health Center a discouraging

experience. Ethically speaking, it is incumbent that the system ensures

patient privacy and maintains doctor-patient confidentiality. On the societal

level, however, this project can be expanded so as to be deployed in

private clinics and, ultimately, overcrowded slow-moving public hospitals

all around the country.

Hospital Management System by Sawant et al., (2019). The

purpose of the project entitled as “Hospital Management System” is to

computerize the Front Office Management of hospitals to develop

software which is user friendly, simple, fast, and cost – effective. It deals

with the collection of patient’s information like add patient, update patient,

delete patient, search patient, view patient diagnosis, etc. Traditionally, it

was done manually. The main function of the system is to register and

store patient details and doctor details and retrieve these details as and

when required, and also to manipulate these details meaningfully. The

Hospital Management System can be entered using a username and

password. It is accessible by an Admin, Doctor & Receptionist. Only they

can add data into the database. The data can be retrieved easily. The

data is well protected for personal use and makes the data processing

very fast.

The Government of India has still aimed at providing medical

facilities by establishing hospitals. The basic working of various hospitals

in India is still on paper as compared to hospitals in European countries

where computers have been put in to assist the hospital personnel in their

work. The concept of automation of the administration and management of

hospitals is now being implemented in India also, with large hospitals like

APOLLO and AIIMS in Delhi, ESCORTS in Chennai, having automated

their existing system.

Computers are not only used to increase the efficiency in all fields

ranging from fixing the appointment with the Doctor to keeping the record

of the Patient.

Since we are entering details of the patients electronically in the”

Hospital Management System”, data will be secured. Using this

application, we can retrieve a patient's history with a single click. Thus,

processing information will be faster. It guarantees accurate maintenance

of Patient details. It easily reduces the book keeping task and thus

reduces the human effort and increases accuracy speed. Hospital

Management System is essential for maintaining detail about the Doctor,

Patient, Hospital staff etc. we understand that by using the Hospital

Management System project the work becomes very easy and we save a

lot of time. Hospital administrators would be able to significantly improve

the operational control and thus streamline operations. This would enable

it to improve the response time to the demands of patient care because it

automates the process of collecting, collating and retrieving patient

Hospital Management Information Systems in Health Sector

and Development in Turkey by Demirel (2018). Information systems

play an important role in the production, sharing, storage and transmission

of information in various fields. Hospital information management systems

used in the health sector actively meet the needs of physicians,

administrators, and patients in institutional processes. Hospital information

management systems ensure that data are collected in a correct,

complete and interdependent manner. Hospital information management

systems started to be used in the 1960s. These systems can be classified

as integrated and modular systems. Integrated information systems are

divided into hospital general information systems, clinical information

systems, management information systems, and a database management

system. In another classification, hospital management information

systems are seen to include resource utilization and programming,

financial management, materials and facility management and staff

management systems. Hospital management information systems have

purposes such as providing patient satisfaction, strengthening internal

communication networks, reducing costs, and reliably storing data in a

digital environment. The development of hospital information management

systems in Turkey is based on the Health Information Systems Project,

18
SOUTH EAST ASIAN INSTITUTE OF TECHNOLOGY
COLLEGE OF INFORMATION AND COMMUNICATION TECHNOLOGY
National Highway, Crossing Rubber, Tupi, 9505
South Cotabato, Philippines

which was supported by the World Bank in 1990 and conducted by the

Ministry of Health. The use of hospital information systems in health

institutions has become increasingly widespread with the studies

conducted under the leadership and coordination of the Ministry of Health

after 2000 years. The purpose of this study is to evaluate the development

and functions of the hospital information management systems in the

health sector by acting from the Turkish case. The study first examined

the development and types of hospital information management systems.

The purposes of the usage and benefits of these systems have been

referred to in the study. In the second part of the study, the development

and components of hospital management information systems in Turkey

are evaluated.

Hospital information management systems are systems that collect

data from different sources, taking on functions of gathering information in

hospitals and transmitting information to administrative units. Such a

system consists of software, hardware, and the necessary communication

infrastructure and is established both on paper and computer. It permits

the efficient exchange of data with doctors and other hospital personnel

with colleagues in and out of their own institutions. Hospital information

management systems have made progress since the 1960s. These

systems, which focused on reducing costs in the 1970s, aimed at bringing

together various elements in a holistic way in the 1980s. According to this;

all detailed information about patients is kept in a system. In this way,

19
SOUTH EAST ASIAN INSTITUTE OF TECHNOLOGY

physicians can make accurate diagnoses by reaching the information

about the patients instantly. This situation offers an advantage in terms of

cost and time by avoiding unnecessary medical tests to be applied to the

patient. The use of electronic patient records became widespread in the

1990s. After the 2000s, it was aimed to increase the quality of patient care

in the health sector through technological applications. Hospital

Management Information Systems are classified as integrated and

modular. Another classification incorporates staff management systems

into hospital information systems with resource utilization and

programming, financial management, materials and facility management.

Management of public healthcare facilities in the

Philippines: Issues and Concerns by Dela Cruz et al., (2019).

Public hospitals are the primary means of healthcare delivery in

developing countries. Given the pressing need for efficient health

services, it is imperative to know the extent to which a country's

public healthcare institutions meet an ever-increasing public

demand. This study aimed to assess the state of hospital facilities

among public health care institutions in a developing country. The

results of this study show that most concerns stemmed from the

lack of financial resources, materials, equipment and technological

innovations; insufficient knowledge, skills and human resources;

and problems that related to processes and methodologies. Public

hospitals are in dire need of facility upkeep to maintain their

operations. This has become a more pressing concern because of

the very limited resources at the disposal of public hospitals. This

study also highlighted the crucial role played by the national

government in finding effective and efficient ways to address these

issues and concerns to ensure successful delivery of healthcare

services in the country.

The Quality of Healthcare System of Level 1 Hospitals in

Northern Negros Occidental by Geroso et al., (2020). Healthcare

is a primary concern of everyone. The United Nations Sustainable

Development Goals recognize the importance of healthcare,

ensuring healthy lives for all. Hence, healthcare organizations

worldwide are interested in achieving better quality and

performance. The Philippines supports this thrust towards

achieving universal health coverage. The Philippine Health Agenda

for 2016 to 2022 focuses on recognizing the advancement and

protection of public health. It has to discourse not just the universal

and environmental dimensions of health concerns. The enactment

of the Local Government Code provided that local government units

deliver public health services with the national government's

technical assistance through the Department of Health (DOH).

Thus, this study describes and compares the level of quality of

hospital healthcare system of Level 1 Hospitals in Northern Negros

Information technology systems in public sector health

facilities in developing countries: the case of South Africa by

Cline et al., (2018). The public healthcare sector in developing

countries faces many challenges including weak healthcare

systems and under-resourced facilities that deliver poor outcomes

relative to total healthcare expenditure. Global references

demonstrate that information technology has the ability to assist in

this regard through the automation of processes, thus reducing the

inefficiencies of manually driven processes and lowering

transaction costs. This study examines the impact of hospital

information systems implementation on service delivery, user

adoption and organizational

culture within two hospital settings in South Africa. The research

provides insight into the reasons for investing in system

automation, the associated outcomes, and organizational factors

that impact the successful adoption of IT systems. In addition, it

25

finds that sustainable success in these initiatives is as much a

function of the technology as it is of the change management

function that must accompany the system implementation.

Ninety-four interviews with doctors, nurses and hospital

administrators were conducted in two public sector tertiary

healthcare facilities (in two provinces) to record end-user

perceptions. Structured questionnaires were used to conduct the

interviews with both qualitative and quantitative information.

Noteworthy differences were observed among the three sample

groups of doctors, nurses and administrators as well as between

our two hospital groups. The impact of automation in terms of cost

and strategic value in public sector hospitals is shown to have

yielded positive outcomes with regard to patient experience,

hospital staff workflow enhancements, and overall morale in the

workplace.

The project chosen the Software Development Life Cycle (SDLC). It is providing

a standard project management framework that can improve the quality of information

systems. This tool provides a framework for guiding developers in the process of

software development. The concept of following consistent project management

framework to boost quality outcomes can be applied equally to healthcare improvement.

Planning – The first phase aimed to find the core problems and constraints occur on

the running system and to formulate goals of analyses construction and system

development that focuses on Hospital Healthcare. The researchers conducted

interviews and investigations with the hospital personnel to gather data and

requirements for the project. The researchers were provided with necessary documents

such as sample of printed forms and receipts of the hospital. In prior planning stage,

researchers observed and collected data at Adventist Medical Center-Iligan Tupi

branch.

Analysis – The researchers analysed the company’s management, workflow, looked for

problems occur within the hospital, healthcare and records. System analysis aims to

find the ideal form of application researcher built, by taking into account various factors

of issues and needs that exist on the system as specified in system planning.

Design – Since the data being gathered and analysed from the previous stage was

fixed and organized, the researchers planned the design of the project based on the

gathered requirements and data. They started designing the database layout for the

system, carefully checking that all is needed for the database is met.

Implementation – The next is implementation. Researchers have planned, to interpret

or translate the system application design into programming language that can

understand by computer system so the application can run and used. At this point, the

researchers had started the coding and building of the system. They also created the

diagrams and flowcharts for the system modules. The administrator, doctor, receptionist,

head nurse, pharmacist, laboratory, and billing officer modules were designed and the

workflows were visualized through flow charts. Researchers used HTML, CSS and

JavaScript for front end designing and PHP, MySQL for database and framework in

visual web implementation displayed.

Testing – Before the program tested, the program must free from error; free from errors

is a necessary testing to find errors that may occur as in the language errors, logic

errors and error analysis program. Each module was tested and the researchers

encountered that there were certain modules which had errors, and the researchers

resulted to fix the errors. The researchers conducted a test plan which allowed the

users to test the existing modules if it’s working or not. The researchers allowed the

user to use the system in order to determine if all the functions were running, and that

the objectives of the project were met.

Maintenance – After the deployment of the system on the health care services of

Adventist Medical Center -Iligan Tupi Branch, maintenance is needed on the system, if

any issue comes up and needs to be fixed or any enhancement is to be done the

developers will be taken care of it.
