Subject: Launch of Healthway Heroes Recognition Program

At Healthway Medical Network (HMN), we believe that our mantra, “Care Beyond Cure” and our
Core Values on Excellence, Integrity, Innovation, Collaboration, Commitment and
Compassion are essential to who we are as an organization. Being members of HMN, we are
expected to embrace this mantra and bring these values to life as we perform our duties and
deliver services to our internal and external stakeholders.

In line with this, we are excited to announce the launch of our newest network-wide recognition
program - Healthway Heroes! As an organization that values the well-being and growth of our
employees, we believe that fostering a positive and engaging work environment is extremely
important. With this program, we aim to recognize and celebrate the exceptional efforts and
contributions of our team members.

To ensure that the “Care Beyond Cure” culture and our Core values are imbibed by the entire
organization, we have created the Heathway Heroes Program that will allow us to recognize,
appreciate and reinforce these values through peer-to-peer nominations.

Through this program, we will acknowledge extraordinary initiatives that move the Company
toward reaching our goals and realizing our shared values – efforts that go beyond normal job
expectations; and that are achieved through the demonstration of our Core Values, including how
they are exemplified in the workplace and how they impact the work that we do.

To kickstart Healthway Heroes 2023, we invite you to become Hero Advocates by identifying your
Healthway Hero nominees through the use of our Healthway Heroes nomination form. This form
will serve as the official platform for you to nominate but more importantly, share your valuable
experiences with your colleagues whom you think best exhibit our six HMN Core Values.

We strongly encourage everyone to look back into the year that was and nominate
peers/colleagues who truly demonstrated behaviors that are befitting of a Healthway Hero.
The nomination form will be accessible to all employees starting today, November 3, 2023
(Friday) until November 18, 2023 (Saturday at 11:59 PM). Kindly click on the link or you may
scan the QR code below to access the nomination site. We kindly request that you allocate some
time to provide thorough and thoughtful responses.

Guidelines in Nomination:
1. Any employee (a.k.a. Hero Advocate) may nominate a teammate, peer/colleague,
supervisor, or subordinate, whom they think have exemplified any of the six HMN Core
Values (shown below) along with the specific incident/s or behavior/s and outcomes that
explain why the employee is being nominated as a Healthway Hero. The 6 Core Values
(award categories) are as follows:

Excellence We deliver quality service of the highest standards.

2. All employees except members of the HMN ManCom are eligible for nomination as
Healthway Heroes.

3. There is no limit as to the number of nominations that employees can submit.

4. There will be 2 levels of Healthway Hero winners – one at the BU level (HMC, QM, and
HPI/HCCH) and another at the HMN level. This means that there can be 6 winners (i.e. 1
winner representing each of the 6 Core Values) per BU who will then be advanced to the
HMN level for them to vie for the over-all Healthway Heroes, where the panel of judges
will choose from among the BU winners those who will be recognized as HMN Healthway
Heroes.

5. Starting 2024, we will have 2 nomination periods - one to cover the first half of the year
(Jan-Jun) and another for the second half of the year (July-Dec). This means there will be
2 awarding ceremonies at the BU level but there will still be just one set of over-all
Healthway Hero winners at the HMN-level, to be awarded in December of every year.

6. The panel of judges will be composed of select ManCom members both at the BU and
HMN levels. Management reserves the right to change or add more members to the panel
of judges as it deems appropriate. The criteria for judging will be as follows:

Impact of the Act/Behavior – 40%

The magnitude of the cited incident, as well as how he/she is innately and genuinely
perceived by others and the environment inside the organization. Consider the magnitude
and impact of the behavior exhibited.

Degree of Involvement – 30%

The employee’s contribution in his/her example is key to the resolution of the situation,
reflected in the ability and the creativity in finding a solution to challenges encountered
with respect to the core value the candidate has been nominated for.

Quality and Consistency – 30%

The relevance of the actions and approach of the nominee to the core value for which
he/she has been nominated for, as well as consistently demonstrating them in his/her
interactions with internal and external stakeholders.
7. Awarding of Healthway Heroes at the BU level will be scheduled by their respective BU
HR Heads while the over-all HMN Healthway Hero winners will be awarded during the
HMN Year End (Christmas) Party in December.

IMPORTANT NOTE:

The HMN Management Team reserves the right to change the overall guidelines, criteria, and schedule of the
Healthway Heroes Program. HMN may also choose not to award, to withdraw, and/or forfeit any of the Healthway
Hero awardees should the participants or entries fail to meet a certain requirement and as it deems appropriate.
