THIS IS TO CERTIFY THAT, the undersigned Marine Surveyor of PT. SIRIUS Maritime Services, Batam,
did, upon at the request of Messrs.,PT. SEASCAPE SURVEYS INDONESIA, reference to your
instruction dated on October 19th, 2023, we were attended on board,
DSV. “ENDEAVOUR”
Port of Registry: TanjungPriok
On October 22nd, 2023 at 14.20hours up to at 23.30hours local time, whilst she was alongside at Tanjung
Wangi Port of Banyuwangi, Indonesia for the purpose of conducting a Bunkering survey 250,000 liters of
solar fuel supplied and we were report as follows:

2.0. GENERAL INFORMATION
The ”ENDEAVOUR” is a Diving Support Vessel with GRT 2397 Tons and under registry of
TanjungPriok was alongside at Tanjung Wangi Port of Banyuwangi Port, Indonesia for the
purpose carry out a loading Solar there about around 250 KL or equivalent with 250,000 Litter’s.
3.0. ATTENDING PARTIES
Present at time of Inspectionbunker on board “ENDEAVOUR” on October 22nd, 2023 were as

follows:

Mr. FatchurRochim : Chief Engineer of DSV. ENDEAVOUR

4.0. INSPECTION
4.1 Upon received instruction dated onOctober 19th, 2023, we have on board “ENDEAVOUR” on
October 22nd, 2023at 14.20 hours up to at 23.30 hours local timewhile she was alongside at Tanjung
Wangi Port of Banyuwangi, Indonesia. After our surveyor on board, we immediately meet Chief
Engineer and explained our purpose of attendance.
4.2 Based on information, we reported that of total cargo discharge carried out by bunkership tanker to
DSV. ENDEAVOUR was amount of 250 KL or equivalent with 250,000 Litter’s.
4.3 As per Chief Engineer planning, the supplied Solar was nominated in fuel tank No. 1 P/S, No. 2
P/S/C, No. 3 P/S, No. 5 P/S and No. 6 P/Sof vessel. Chief Engineer was carried out sounding to all
Fuel Tank of DSV. ENDEAVOUR by means of sounding tape. The received quantity was
ascertained by ship tank calibration.

5.0. QUANTITY OF BUNKERING
5.1. Quantity Marine Diesel Oil on Board

Based on result of actual sounding us, we were determined the quantity of bunker each
ship’s tank. The calculation of cargo was ascertained by means of ship’s tanks calibration
table. We reported the on board quantity of Solar as follows:
