AFFIDAVIT
I, xxxxxxxxxxxxx, of legal age, Filipino, and currently residing at
xxxxxxxxxxxxxxxxxxxx City Philippines, after having bee duly sworn to an oath in
accordance with law, hereby depose and say:
1. I acquired my Tax Identification Number (TIN) from BIR Region X, Cagayan de
Oro City, Philippines.

2. Thus, I was issued TIN xxxxxxxxxxxxxxxx, by the aforementioned Office.

3. Just recently, I was able to land a job at xxxxxxxxxxxxxxxx Parañaque City.

4. Considering that my work place is now in Parañaque City and I am currently

residing in Caloocan City , my employer needed to transfer my TIN accordingly.

5. However, I was informed that when my new employer tried to process my TIN
transfer, they were informed by BIR that my TIN is connected to a business or
that I own a business in xxxxxxxxxxxxxxxx.

6. I was surprised because I never owned a business or even started a business in

Cagayan de Oro City, hence the said business reflecting in the system of BIR
Region X, Cagayan de Oro City does not exist.

7. From the forgoing, I would like to request from BIR Region X, Cagayan de Oro
City to update my records and allow the transfer of my TIN from Cagayan de Oro
City to the proper BIR Office/Region which covers my new place of residence
and/or employment whichever is applicable.

8. I am executing this affidavit to attest the truth of the foregoing and to support my
application for updating BIR Records and transfer of my TIN, and for whatever
legal purpose this may serve.
