    being sworn to in accordance with law, depose and state that:

    1. I am the owner of a Facebook Account named: xxxxxxxxxxxxxxxxxxx.

    2. On xxxxxxxxxxxxxxxxxxx, I received messages from family and friends that they


    received messages from my Facebook account that are very unlike me.

    3. Thereafter, I tried to open my Facebook Account but I could no longer do so.

    4. Somebody must have been able to access my Facebook Account and change the
    password thereof.

    5. Any person who has received a message, call or any communication through the
    aforementioned Facebook account from xxxxxxxxxxxxx onwards, said
    communication is not from, nor did I authorize such.

    6. Finally, I attest to the truth of the foregoing statement and to deny categorically
    and specifically under oath that I am not the person who is using the Facebook
    Account xxxxxxxxxxxxxxx from xxxxxxxxxxxxx onwards.
