JOINT AFFIDAVIT
(OF TWO DISINTERESTED PERSONS)

We ROSALYN B CAMPO, of legal age, married, Filipino citizen, and PASCUALA B.

CALANO, of legal age, widow, Filipino citizen and both residents of Lattac, Sto. Niño, Cagayan,
after being duly sworn to law, hereby depose and say:

1. That we know personally the late NICANOR BINUYA VALENZUELA of Lattac, Sto.
Niño, Cagayan;
2. That said person died on April 29, 2001 at Lattac, Sto. Niño, Cagayan;
3. That we know about the death of said person for we were then present during the wake
and we even attended his burial services;
4. Unfortunately, due to inadvertence, said person’s death was not immediately registered
to the Local Civil Registry of Sto. Niño, Cagayan at the time of his death or immediately
thereafter, hence, the non registration of said person’s death;
5. That we know all about the afore-stated facts for we are close neighbors in Lattac, Sto.
Niño, Cagayan for a long time;
6. That we are executing this affidavit to attest to the fact that said NICANOR BINUYA
VALENZUELA really died on April 29, 2001 at Lattac, Sto. Niño, Cagayan.

FURTHER, AFFIANTS SAYETH NONE.
