    Ma’am/Sir:

    Good day!

    I am a student at Davao del Sur State College (DSSC), taking a Bachelor of


    Technology and Livelihood Education Major in Home Economics. I am writing this
    letter to ask permission from you, the Punong Barangay to allow me to gather
    data from children ages five to eight years old from the archives of Barangay
    Health Workers in your Barangay. The data I will acquire will be used as part of
    our requirement in the subject, Food and Nutrition TLHF 312. The data I need
    includes the children’s names, sex, age, height, and weight. The data will help
    me monitor their body mass index which will last for two (2) months.
