    PATRIOTIC & CIVIC AWARDS

    A. Objective:

    1. To achieve the prestigious and highest Patriotic and Civic Awards this
    Fraternal Year 2022 – 2023 in District – 21, Luzon North Jurisdiction,
    Philippines and Supreme levels.

    B. Entries:

    1. Patriotic Award - 500 Years of Christianity in the Philippines Coffee Table Book
    2. Civic Award - Oratorical Contest plus Three(3) Socio – Civic Activities

    C. Requirement:

    1. Submission of the following documents to Supreme and District-21

    a. Form 186 - Elected Officers of the Chosen Year


    b. Form 1315 - Annual Audit Report for FY – 2021 – 2022
    c. Form 1728 - Annual Survey Report for CY 2022

    2. Membership Growth:

    a. Seven (7%) of Membership Roster of FY 2022 – 2023

    3. Socio & Civic Activities:

    a. Minimum of Four (4) programs and projects

    4. Minutes of Assembly Meetings and/or Newsletters

    a. Minimum of Six(6) copies as recorded

    5. Additional Requirement:

    a. Project Write Ups


    b. Photos & Other Documents

    6. Deadline of Entry:

    a. April 30, 2023
