documents and records contained and attached in the application *
2. Description of waste management plan *
3. Pollution Control Officer Accreditation Certificate *
4. Contingency and Emergency Plan *
5. Photographs of Hazardous Waste Storage Area *
6. Official Letter of Request *
7. Mass Balance of Manufacturing process
8. Analysis of waste(s)
9. Other relevant Information (e.g., planned changes in production process or output, comparison
with related operation)
10. Copy of Environmental Compliance Certificate (ECC) or Certificate of Non-Coverage (CNC)
11. Copy of Valid Permit to Operate (PTO)
12. Copy of Valid Discharge Permit (DP)

ADDITIONAL REQUIREMENTS FOR APPROVED CLUSTERED

ESTABLISHMENTS
1. List of individual tenants or establishments.
2. Information on the individual member establishments per approved cluster
3. Letter from EMB Central Office on the approved clustering
4. Affidavit of Joint Undertaking among individual member establishments, the cluster
Managing Head and the cluster PCO
5. Map of clustered individual establishments including geotagged photos of the façade of
the establishments
