R2TMC TRAUMA CARE PATHWAY

EMERGENCY ROOM TRAUMA TEAM ACTIVATION


DECISION ALGORITHM
ADULT (≥ 16 YEARS OF AGE) – ANY TRAUMA MECHANISM
Is the patient known or suspected of having one or more of the following
(Choose from the following)

PHYSIOLOGICAL CRITERIA ANATOMICAL CRITERIA


 All penetrating injuries to the head, neck or torso
 All penetrating injuries to the extremities proximal to the elbow/knee with
 Respiratory distress, hypoxia or a need
significant blood loss
for airway control/intubation
 Major burns > 20% BSA with trauma
 GCS ≥ 13
 Open or depressed skull fractures
 RR <10 OR >30
 New paralysis or neurological deficits
 SBP < 90 mmHg
 Facial injuries with potential airway compromise
 Two or more proximal long bone fractures
 Crushed, degloved, mangled or pulseless extremities proximal to the wrist or
ankle
 Amputation proximal to the wrist or ankle
 Chest wall instability or deformity
 Unstable pelvis

Was there a major mechanism of injury within 24 hours?


(Choose from the following)
 Fall ≥ 6m (20ft)  Pedestrian, pedestrian conveyance  Penetrating injury
 Motor Vehicle Collision or bicyclist struck/collision (i.e. firearm Or
 Vehicle Speed > 65km/h  Vehicle speed >30km/ stabbing)
 Any vehicle intrusion including roof:  Fixed object with significant  Blasts and/or
≥ 0.3 meters on occupant side momentum explosions
≥ 0.5 meters any side  Thrown > 3m (10ft)  Significant assault
 Ejection (partial or complete)  Run over
 Death in same passenger compartment
 Roll over
 Motorcycle or ride-on vehicle collision:
 Vehicle Speed > 65km/h
 Ejection/separation from vehicle

 Health care provider discretion
 Minor trauma mechanism with physiological/anatomical signs
ACTIVATE 

Major mechanism alone
Pregnancy > 20 weeks
TRAUMA TEAM  >55 years with low impact mechanism
 SBP < 110mmHg >6 years
 Co-morbidities or pre-existing medical conditions
 Anticoagulation
 Auto Launch or Early Fixed Wing Launch (EFWL) from scene
 Burns >20% BSA (without trauma)
