Acupuncture
The 1997 National Institutes of Health consensus statement concluded that
acupuncture may be useful adjunct therapy or as an acceptable alternative
treatment of CTS.32
A more recent systematic review from 2016 also confirms these findings.33
Acupuncture treatment has been shown to be as effective as oral steroids for
CTS over a 4 week period and this effect seems to be sustained over a 1-year
follow-up period.34,35
When compared with wrist splinting, patients treated with acupuncture report
fewer symptoms and improved physiologic measures with improved nerve
conduction velocities.36

Electroacupuncture (EA) is a complementary therapy to improve morphine analgesia for •
postoperative pain, but underlyingmechanism is not well-known. Herein, we investigated
EA-induced analgesic effect in a plantar incision (PI) model in male Sprague-Dawley rats.
PI was performed at the left hind paw. EA of 4 Hz and high intensity or sham needling was
conducted at right ST36 prior to PI and repeated for another 2 days. Behavioral responses
to mechanical and thermal stimuli, spinal phospho-ERK, and Fos expression were all
analyzed. In additional groups, naloxone and morphine were administered to elucidate
involvement of opioid receptors and for comparison with EA. EA pretreatment significantly
reduced post-PI tactile allodynia for over 1 day; repeated treatments maintained analgesic
effect. Intraperitoneal naloxone could reverse EA analgesia. Low-dose subcutaneous
morphine (1 mg/kg) had stronger inhibitory effect on PI-induced allodynia than EA for 1 h.
However, analgesic tolerance appeared after repeated morphine injections. Both EA and
morphine could equally inhibit PI-induced p-ERK and Fos inductions. We conclude that
though EA and morphine attenuate postincision pain through opioid receptor activations,
daily EA treatments result in analgesic accumulation whereas daily morphine injections
develop analgesic tolerance. Discrepant pathways and mechanisms underlying two
analgesic means may account for the results

Acupuncture is effective for the treatment of chronic pain and is
therefore a reasonable referral option. Significant differences
between true and sham acupuncture indicate that
acupuncture is more than a placebo. However, these
differences are relatively modest, suggesting that factors in
addition to the specific effects of needling are important
contributors to the therapeutic effects of acupuncture

Implications in Management of Migraine

We investigated the effectiveness of acupuncture in childhood migraine in 22 children


with migraine, randomly divided into two groups: a true acupuncture group (12
children) and a placebo acupuncture group (10 children).
Ten healthy children served as a control group. Opioid activity in blood plasma was
assayed by two methods: (1) determination of total (panopioid) activity with an
opiate radioreceptor assay, and (2) determination of [~-endorphinlike
immunoreactivity by radioimmunoassay. The true acupuncture treatment led to
significant clinical reduction in both migraine frequency and intensity.
At the beginning of the study, significantly greater panopioid activity was evident in
plasma of the control group than in plasma of the migraine group. The true
acupuncture group showed a gradual increase in the panopioid activity in plasma,
which correlated with the clinical improvement.
After the tenth treatment, the values of opioid activity of the true acupuncture group
were similar to
those of the control group, whereas the plasma of the placebo acupuncture group
exhibited insignificant changes in plasma panopioid activity.
In addition, a significant increase in [~-endorphin levels was observed in the migraine
patients who were treated in the true acupuncture group as compared with the
values before treatment or with the values of the placebo acupuncture group.
The results suggest that acupuncture may be an effective treatment in children with
migraine headaches and that it leads to an increase in activity of the opioidergic
system.

Limited evidence suggests that acupuncture may have some antipsychotic effects as
measured on global and mental state with few adverse effects. Better designed large
studies are needed to fully and fairly test the effects of acupuncture for people with
schizophrenia.
