PARENTAL CONSENT FORM

FOR FOOTBALL TOURNAMENT

I,______________________________ (name of parent/guardian), certify that I am the

parent or legal guardian of ______________________________ (name of participant), whom I am
allowing to actively participate in High Unite’s Football Tournament on October 23, 2023 at Valle
Verde 5 Clubhouse, Pasig City. In view of this, I do hereby acknowledge, consent, and agree to the
following:

● I am aware that participation in this face-to-face event is completely voluntary and I have
been fully informed of the various programs to be conducted.
● I declare that my child is physically fit and prepared for this event. If applicable, all health or
physical-related issues of my child have been declared to the High Unite organizers.
● I understand that participation in this event involves certain risks associated with in-person
events and gatherings including, but not limited to, contraction of viruses, physical injuries,
and other accidents.
● I hereby agree to release, defend, indemnify, and hold harmless Homeschool Global, its
partner schools, High Unite, and any of its officers or volunteers from any and all claims for
any and all expenses, personal injuries, losses, or damages incurred or caused by my child’s
participation in the event.
● I am aware that my child cannot attend this event without a signed copy of this consent form
and may be barred from entry if they fail to present this to the event staff.
● I hereby attest that I have read (or have had it read to me) this consent form. I understand it
and I have signed it voluntarily.
