PARENT/GUARDIAN CONSENT FORM FOR SYMPOSIUM

This parental consent form is provided to both inform you and to request your permission
for your child/children to participate in a symposium to be held on October ___, 2023. The
symposium will be a part of their final performance task for the 1st Quarter. During the said event,
students are required to be present at school at exactly 1:30 PM – 4:30 PM. For the duration of the
event, participants are expected to act in a school-appropriate manner. Regular school rules and
consequences will apply. In the event of inappropriate behavior, a student may be removed from
the event and disciplinary action may be taken.

As the parent/guardian of ___________________________________, I hereby acknowledge that

I have been informed of the details of the school activity. Furthermore, I understand the risks
associated with an school activity and agree that the rules and regulations established for the said
activity are for the safety and security of the participants, and thus agree to instruct my child to
obey them.

Having understood all of the aforementioned:

I am allowing our child to join and participate in the Symposium conducted by HSNHS
Grade 12 teachers. I/We understand that in choosing to join each such conference, our
child is agreeing to act in accordance with the rules/policies/procedures of Holy Spirit
National High School.

I am not allowing my son/daughter to join video/audio conferences conducted by

HSNHS Grade 12 teachers.
