2012 Annual Enrollment for Fujitsu Benefits


October 18 - 30, 2011
Annual Enrollment for 2012 Fujitsu Employee Benefits is less than a month away. Over the coming weeks, you will receive a series of Sharing News communications designed to help you consider your 2012 benefit options and choose the coverage that is right for you and your family. In this issue: Sharing News #1 shares highlights of 2012 benefit changes, including the introduction of the redesigned Kaiser CA HMO plan, the Kaiser CA Deductible HMO plan (DHMO), and gives you a heads up about more information coming your way soon. In the next issue: Sharing News #2 reviews your 2012 benefit choices, lists your 2012 premiums for the medical and dental plans and provides resources to learn more about the Kaiser CA DHMO.

Health Care Consumerism, Fujitsu and You

Youve become better health care consumers, using your benefits wisely and making healthy choices. The more you improve your health, the lower health care costs are for everyone. The decisions you make today impact, not only your physical health, but your financial health tomorrow. Fujitsu is committed to managing our benefit programs to keep them more affordable for both you and the company. We listen to your feedback and try to find ways to provide the coverage you want, while at the same time making practical decisions that allow us to manage rising health care costs. We appreciate the effort that you have made to be conscientious health care consumers and need you to continue to better manage your own health. When it comes to making benefits decisions during Annual Enrollment, its important that you consider your options carefully and decide what is best for you and your family.

2012 Benefit Changes Coming Your Way


Here is a quick overview of whats happening with your 2012 benefits. More details to follow in the coming weeks: Anthem Lumenos HSA Plan: the amount you can contribute to your Health Savings Account is increasing. Your total HSA contribution limits for 2012 are: $3,100 for employee only $6,250 for employee + one dependent $6,250 for employee + two or more

Starting in 2012, the Anthem Lumenos HRA Plan will not pay secondary claims. If your spouse/domestic partner or child(ren) has primary coverage under another medical plan, the Anthem Lumenos HRA plan will not coordinate benefits coverage with that plan. The Anthem Lumenos HRA plan no longer pays secondary claims. However, you may still receive a Coordination of Benefits (COB) inquiry letter and be required to provide information regarding other coverage. Please note: You and your spouse/domestic partner may not need to cover yourselves and/or your child(ren) under both medical plans. Since loss of medical coverage for a dependent or yourself (if you are not enrolled in our plan) is a Qualifying Status change, you and/or your dependents may enroll in the Fujitsu medical plan within 31 days of loss of the other coverage. See the Medical Summary Plan Description posted at https://fujitsu.employee.com for a complete list of Qualifying events or call the Fujitsu Benefits Service Center at 1-800-552-5989. Reminder: If both you and your spouse/domestic partner cover your dependent children, the primary payor plan continues to be determined by the date of birth of the parents. The plan of the parent whose birthday (month and day) falls earlier in the calendar year is the primary plan.

The Kaiser CA HMO plan is changing to a Deductible HMO (DHMO) plan. Members will need to meet an annual deductible before certain services are paid by the plan. Once you have met your Annual Deductible you will pay 10% coinsurance for most services, up to the Annual Out-of-Pocket Maximum. Routine preventive care is covered at 100%. Details of the Kaiser CA DHMO plan and information on an upcoming meeting and scheduled webcasts are included in mailings to current members. Upcoming Sharing News articles will also provide resources for learning more about the Kaiser CA DHMO plan.

Short-Term Disability Insurance pre-existing condition limitations eliminated for new hires beginning January 1, 2012. If you are a new hire in 2012 and enroll in the plan when you are first benefits eligible (hire date) you are not subject to pre-existing condition limitations. If you were a new hire in 2011 and enrolled in the plan when you were first benefits eligible (hire date) the pre-existing limitations will not apply to you in 2012. If you are a current employee who enrolls in the plan for the first time during Annual Enrollment you are still subject to the pre-existing condition limitations in 2012.

Long-Term Care Insurance plan administrator is changing in 2012. Prudential will replace MetLife as the Long-Term care plan insurer in early 2012. Information regarding this transition will be available in 2012.

Dont Miss These Important Reminders


There were several plan changes last year, some due to health care reform, that you might want to review again before making your benefit elections for this year. Below is a list of these changes: Behavioral Health benefits are treated like other medical benefits and are included in the same limits for deductible, co-insurance and out-of-pocket maximums under each of the medical plans. Behavioral Health benefits for: Lumenos HRA plan participants are provided through the Magellan Behavioral Health Plan. Lumenos HSA plan participants are provided through the HSA medical plan and are managed by Anthem Behavioral Health. Kaiser CA and Kaiser HI medical plan participants are provided through the Kaiser medical plan. HMSA plan participants are provided through the HMSA medical plan.

Lumenos HRA and HSA plan participants can purchase a 90-day supply of prescription drugs through either mail order or retail. (You may still purchase a 30-day supply through retail.) To check pricing go to www.anthem.com/ca. Just a reminder, Express Scripts began managing prescription drug benefits for the Anthem Lumenos Plans on July 1. Amounts paid for over-the-counter drugs do not qualify for reimbursement from your Health Care FSA or Lumenos Health Savings Account (HSA). For additional information go online or contact the Internal Revenue Service and ask for Publication #502. You may also contact Conexis at 1-866-279-8385 or www.conexis.org for assistance. Fujitsu continues to verify the eligibility of newly enrolled dependents in the medical and dental plans.

Enrolling a Spouse, Domestic Partner or Dependent Children


Dependent Social Security Numbers (SSNs) are required for spouses and domestic partners. The Fujitsu Health Plans require Social Security numbers (SSNs) for enrolled spouses and domestic partners. Note: Employees who are exempt from Medicare withholding from their pay (such as a majority of assignees or individuals in the U.S. on certain types of VISAs) are not required to provide the SSN of their spouse or domestic partner. Employees with spouses or domestic partners on certain VISAs may be exempt from providing a SSN. If your spouse or domestic partner is exempt, (and you are not an assignee) you must contact the Fujitsu Benefits Service Center to activate the exemption from the SSN requirement for 2012.

Time to Update Your Life or Supplemental Long-Term Disability Insurance?


Supplemental Life Insurance plan rates for employee and spouse are decreasing for 2012. Reminder: Your rate may increase if your age moves you into a new age band. Bands are broken into five-year segments. Enrollment in Supplemental Life, Supplemental Accidental Death and Dismemberment (AD&D) and Supplemental Long-Term Disability (LTD) insurance is available during Annual Enrollment or after a Qualified Change in Status event, or within 31 days of hire.
