PARENTAL CONSENT

I/We hereby willingly and voluntarily give consent to the participation of

my/our son/daughter _____________________________ in the CAPSAA
Literary-Musical Competitions to be held on November 17, 2023 at Sumulong Memorial
High School, Antipolo, Senior High School Building from 8:00 am to 12:00 nn.

I/We have considered the benefits that my son or daughter will derive from his/her
participation in this activity provided that due care, diligence, and necessary precautions
will be observed to ensure his/her health and safety.

We required student to be accompanied by 1 parent/guardian on the said date. A logistic

service is available in school with a fee of 50.00 per head.
