I, CCC, Filipino, married, of legal age, with office address at CCC Law Office, Nueva
Ecija, do hereby state under oath that:

1. I am the counsel of AAA, Filipino, of legal age, and with residence at Nueva Ecija.

2. I have personally verified her residency at the abovementioned address. I attest that she
has been presently residing thereat for over six (6) months already prior to the filing of
this Petition for Declaration of Nullity of Marriage against the Respondent.

3. I am executing this Certification in compliance with OCA Circular No. 63-2019.
