Do solemnly undertake to cooperate, participate and do the following:

1. Assist my child to meet and comply with school rules and regulations namely:
1.1 wearing of School uniform (female: blue skirt and white blouse, black shoes and
white socks; male: white shirt jack with EVSU logo, navy blue slacks, black shoes
and white socks); appropriate haircut and wearing of School I.D.;
1.2 regular attendance
1.3 regular payment of required school fees
1.4 Observance of proper conduct and other relevant regulations

2. Attend P.T.A. meetings, parents’ conferences and other occasions when my presence
is of most importance for the interest and welfare of my student. (Bring up to GPTA
on the sanctions of non-attendance)
3. Encourage my child to work satisfactorily or very satisfactorily if not excellent class
performance.
4. Coordinate, assist and connive for social, emotional and moral development of my
child together with the teachers, guidance and counselors, school administrators and
other school personnel who are most concerned with his/her welfare.
In default/ neglect, the office has the legal authority to put my child on probation and
may result to his/her non-admission to this school for the next school year.
