1. That I Marilyn Ofredo Alvaro execute this manifestation for the purpose of
the Certificate of Declared Legal Beneficiaries;

2. I am using my father’s surname (Alvaro) in lieu of their supposedly

contracted marriage however upon verification from Philippine Statistics
Authority (PSA), their marriage was not duly registered;

3. That my father was deceased on January 17, 2010, hence, registration of

their marriage will not be possible;

4. Thus, the undersigned declared my mother (Puriza Ofredo Alvaro) as my

beneficiary, attaching the original copy obtained from the Philippine
Statistics Authority (PSA);
