    OutPatient Allocation of 5,000 pesos.

     This allocation is on top of your medical and dental benefits.


     This can be utilized through company charging or by
    reimbursements, like laboratories, medicines, and consultation
    with your private doctor.
     For the reimbursement process, please coordinate with the
    OHN and submit the following:
    Signed PGB OPD reimbursement form
    Original official receipt
    Copy of prescription (for medicines)
    Copy of laboratory request (for laboratory/diagnostic
    test)
    Medical certificate
     The requisition will be forwarded to management for approval
    every second and fourth Friday of the month.
     Lead time for crediting of refund will depend on the timeline
    per affiliate.
     Exclusion: multivitamins, aesthetic-related procedure that
    do not pose any medical emergencies/concerns, pregnancy
    related treatment or procedures.
