CONTRACT OF LEASE

KNOW ALL MEN BY THESE PRESENTS:

This agreement is made and executed by and between:

WITNESSETH;

WHEREAS, the LESSOR is the owner of the LEASED PREMISES. a residential/commercial property
situated at

WHEREAS, the LESSOR agrees to lease-out the property to the LESSEE and the LESSEE is willing to lease
the same;

NOW THEREFORE, for and in consideration of the foregoing premises, the LESSOR leases unto the
LESSEE and the LESSEE hereby accepts from the LESSOR the LEASED PREMISES, ubject to the following
terms and conditions, to wit:

1. Period and rentals: This Contract of Lease shall be for a period of __________ YEARS from ____ to
____ during which period the LESSEE shall be bound to pay LESSOR a monthly rental of
____________________ PESOS ____________ due and payable every 1 st week of the month thru bank.

Upon its expiration, this lease may be renewed under such terms and conditions as may be mutually
agreed upon by both parties, and thereafter renewable upon mutual agreement of the parties;]

2. Use and purpose: The LESSEE hereby expressly agrees and warrants that the leased premises shall be
used by him exclusively for residential/business purpose and LESSEE is hereby prohibited to use the said
premises for any other purpose or purposes without the written consent of the LESSOR. No subleasing
of the property is authorized unless the LESSOR gives her consent in writing. The LESSEE also agrees
neither to use nor introduce improvements or alterations to the vacant lot at the back of the leased
property;

3. Occupation after termination of lease: After this lease shall have terminated for any reason
whatsoever, if the LESSEE continues to occupy the premises without the consent of the LESSOR, such
extension of lease shall be deemed only tolerated as running from month-to-month under the same
terms and conditions herein stipulated, and may be terminated by the LESSOR by means of a written
notice served upon the other party at least ten (10) days in advance;

4. Repairs and maintenance: The LESSEE hereby expressly acknowledges that the leased premises are in
good tenantable condition and agrees to keep the same in such good and tenantable condition which
purposes he shall bind himself to undertake at his exclusive expense all ordinary repairs necessary to
maintain the same in such condition. The LESSEE also agrees not to change any of the locks/ keys of the
leased property. It is expressly agreed and understood however that the LESSEE shall not commence or
proceed with any such repair work nor in any case Introduce improvements or make any alterations in
the leased premises without the prior consent of the LESSOR. Provided however that all Improvements
or alterations made shall form an Integral part of the leased premises and shall not be removed
therefrom but shall belong to and become the property of the LESSOR upon termination of the lease,
without need of reimbursement for the costs there;

5. Water, electricity, telephone, etc.: Charges for water consumption, electricity, telephone and other
utility services in the premises, as well as for the repairs utility system shall be for the exclusive account
of the LESSEE. It is understood that should the LESSEE fail to pay the utility services within five (5) days
from billing, the said LESSEE authorizes the LESSOR to cut said utility services until the same has been
fully paid;

6. Depositor requirement: Upon signing of this contract, the LESSEE shall pay unto the LESSOR an
amount equivalent to one (1) month rental as advance rental and two (2) months deposit, the same to
be applied in payment of back rentals, if any upon the termination if this Contract and other expenses
and charges billed against the LESSEE for water, electric, telephone and other utility services remaining.
In case of any breach of contract, the deposit shall be forfeited in favor of the LESSOR;

7. Insolvency of the LESSEE: The rentals stipulated shall b􀃈 in arrears or unpaid for at least two (2)
consecutive months, or if the LESSEE shall at any time fail or neglect to perform or comply with any of
the covenants, conditions, or restrictions stipulated, or if the LESSEE shall become insolvent, then in any
such above cases this lease contract shall be automatically terminated and cancelled and the said
premises shall be vacated peacefully by the LESSEE for the LESSOR to take over the possession thereof in
which event it shall be lawful for the latter or any of her representative to enter into and upon the
leased premises as though the terms of this lease contract has expired without prejudice on the part of
the LESSOR to exercise any or all the rights arising from this Contract of Lease and those granted by law;

8. Remedial measure: In case of violation or infringement of any of the foregoing terms and conditions,
the LESSOR reserves the right to terminate this Contract of Lease immediately and the LESSEE agrees to
vacate forthwith the premises without need of court proceeding. Provided however, that if for any valid
reason It shall become necessary for the LESSOR to constitute an appropriate court caption for the
enforcement of her rights under this contract, the LESSEE shall be liable in liquidated damages for and in
lieu of attorney's fees aside from court costs and other legal charges allowed by the Rules of Court;

9. Previous agreements superseded: This contract supersedes and renders void any and all agreements
and understanding, oral or written, previously entered into by the parties covering the leased property;
