1. Based from the assessment, what are the most established learning gaps identified for the
first and 2nd quarter? By:
a. Subject Area? The teacher identified the learning gap in English and Mathematics.
b. MELCS?
Use context clues to find meaning of unfamiliar words: definition, exemplification (EN4V-Ia-31)
Use clear and coherent sentences employing appropriate grammatical structures: Kinds of
Nouns – Mass Nouns and Count Nouns, Possessive Nouns, Collective nouns (EN4G-Id-33)
Solves multi-step routine and non-routine problems involving division and any of the other
operations of whole numbers including money using appropriate problem solving strategies and
tools (M4NS-Ie-45.5)
Multiplies numbers up to 3-digit numbers by up to 2-digit numbers without or with regrouping.

They were not able to understand the lesson because they need their teacher to discuss it in
front of them. Even though the self-learning module is very comprehensive and with more
examples, there is a need for them to have one explaining the concepts.
I gave a module to the learners with more examples and monitored them regularly. For
those who are unable to browse to the social media created and intended for grade 4 pupils
and parents/guardians, I scheduled a home visitation to them. Other learners were being
monitored by calling or messaging them about the status of their work. I asked them on the
concepts that are not yet clear to them and gave comprehensive explanation to them.
