    The physical examination showed a well-developed body that was proportional with equal fat distribution and no deformities. The skin had a brown complexion without lesions or discoloration. The head was symmetric with no abnormalities noted. The eyes, ears, nose, mouth, neck and chest were also examined with normal findings reported. The abdomen had stretch marks and a vertical line but was otherwise normal. The extremities appeared normal without pain or edema.

    Physical Development, Budy Body is proportional. Bilateral muscles are well developed. There
    build & Fat Distribution is equal distribution of fat with some subcutaneous fat. Body
    parts are intact and appear equal without obvious deformities.

    Skin Condition & Color Color is even; The client has brown complexion
    Dress & Hygiene Appropriately dressed

    General Skin Coloration Brown, no obvious lesion and discoloration on extremities

    Skin Integrity Skin is intact

    Temperature Warm
    Mobility and turgor Skin is mobile, with elasticity and returns to original shape
    quickly
    Edema No signs of edema

    Head
    Size, Shape & Configuration Head is symmetric, round, erect, and in midline and
    appropriately related to body size. No lesions are visible.
    Movement No involuntary movements
    Consistency
    Face Symmetric with oval appearance. No abnormal movements
    noted
    Temporal Artery
    Temporomandibular Joint (TMJ) No swelling and abnormalities with movement. Mouth open and
    closes fully and jaw moves laterally in each direction. No pain
    Eyes
    Eyelids & Eye lashes The upper and lower lids close easily and meet completely when
    closed. Without redness, swelling and lesions. Eyelashes are
    evenly distributed.
    Eyeballs Symmetrically aligned in sockets without protruding or sinking.
    Sclera Sclera is white.
    Palpebral Conjunctiva Clear and free of swelling and lesions
    Lacrimal Apparatus No swelling or redness
    Cornea and Lens Cornea is transparent, with no opacities. Oblique view shows a
    smooth and overall moist surface the lens is free of opacities
    Iris and Pupil Iris is evenly colored. The pupils are round, centered to iris and
    equal in size bilaterally.

    Ears

    Auricle, Tragus and Lobule Ears are equal in size bilaterally. The auricle aligns with the
    corner of each eye. Earlobes are free and tightly attached to
    adjacent skin. Skin is smooth, with no lesions, lumps, or
    nodules. Color is consistent with facial color. No discharge
    present.
    Nose
    Appearance Symmetrical and no lesions present
    Nasal Sinuses
    Patency of airflow Client able to sniff through each nostril while other is occluded

    Mouth

    Lips Brownish red, dry without lesions or swelling


    Tongue In center, moist, light pink

    Neck

    Appearance Neck is symmetric, with head centered without bulging masses.


    Color Hyperpigmentation
    Movement No abnormal movements
    Range of motion Neck movements are smooth and controlled.
    Musculoskeletal-Upper Extremities
    Arms and hands Normal, no presence of lesions and edema
    Underarm Hyperpigmentation

    Appearance Breast Tissue and the areas breast look normal and normal in
    size and shape / no presence of swelling
    Heart
    No presence of DOB
    Abdomen
    Appearance Have a Stretch marks and Vertical line in abdomen
    Skin Lighter color compared to exposed skin
    No lesions, Scars and Rashes
    Umbilicus Slightly protruding due to distension of abdomen/pregnancy.
    Musculoskeletal-Lower Extremities
    Appearance Normal and no presence of pain
