Re: Mrs. Anita Ramamurthy(Aged59)

I am writing to request a home visit for Mrs Ramamurthy, a 59-year-old married woman who was
hospitalized due to acute appendicitis on 18/06/2017. She is going to be discharged today.

Mrs. Ramamurthy presented to our hospital with complaints of high fever and intense pain in her
right lower abdomen. After observations, she was diagnosed with the mentioned issue. During
hospitalization, she was treated with IV antibiotics. Moreover, her blood sugar level was
continuously monitored. Her recovery has been encouraging. At this present time, she is stable.
Additionally, an appendectomy surgery is scheduled to be performed 6 weeks later.

In terms of her medical history, she has been suffering from diabetes mellitus type 2. Please note
that she has a sedentary life, and does not exercise. Additionally, she is not compliant with diabetic
medication. Furthermore, she has been diagnosed with essential hypertension.

Mrs. Ramamurthy’s endocrinologist wants her BS level to be regularly charted and prescribed
injections of regular insulin, 8 units bd for one week. Mrs. Ramamurthy has accepted this order but
she is concerned she cannot able to handle the injections by herself. Regarding this issue, it would
be greatly appreciated if you provide assistance to ease this problem. It should be mentioned that a
diabetic and low-fat diet needs to be provided for her.
