INVOICE REQUIREMENTS FOR PAYMENT

1. All invoices to be made out to MVIA. VAT No. 4010261909

2. All amounts authorised are Inclusive of VAT at 15% and include parts and labour.
3. A full detailed invoice (A proper description of the goods or services supplied).
4. Vehicle details and client details stipulated on the invoice.
5. The claim number (XTR063400000021)
6. A serialised invoice number.
7. The service provider’s banking details. Please note that banking details are to be
printed on the invoice or must be sent on the company’s letter head. No hand-written
banking details will be accepted.
8. The service provider’s full address and phone numbers.
9. Please Email all invoices to Payments@MVIA.insure in a secured PDF Format, no
Word, Excel, handwritten scanned copies or any format other than a PDF will be
accepted for payment.
10. All invoices to be submitted to the relevant department before Wednesday 12:30 for
payment on Thursday. If not received by 12:30 payment will be the following

This letter serves to confirm that:

I, Mr ALASTAIR NEWCOMB JAMES COLDWELL, 8305305121084, are fully satisfied with the work
and that I am happy that the money that was approved for the repairs to my vehicle will
now be paid to the approved company.

This document is to be signed by the client only.

If you are not satisfied by the repairs done on the vehicle, please do not sign the release
note and please inform us of the situation on 087 2323 494.
