AFFIDAVIT

I, GIOVANI BALUYOT SANTOS, of legal age, Filipino, and a resident of 4

Overland Drive Sumilang Road Dalandanan, Valenzuela City, after having been sworn to
in accordance with law, do hereby depose and state:

That I am the registered owner of JIVAN’S GARMENTS SUBCONTRACTING

SERVICES located at #88-A Cortez St., Malinta, Valenzuela City with Business
Account No. 20141304;

That however, I was not able to operate the said business, thus will change
ownership to my wife Jinalyn Esparcia Santos;

That at the time of the closure of my business until to date, I did not incur any
obligation from any creditor;

That I am requesting for the cancellation of my Registered Business Name due to

change of ownership;

That I declare under oath that the cancellation of my Business Name registration is
not to defraud creditors. That I have no outstanding and pending financial obligation
with any creditor;

That I am executing this affidavit to attest to the truth of the foregoing facts and for
all legal intents and purposes it may serve, more particularly to inform the
authorities and agency concerned.
