Application form duly accomplished; 2. Certified True Copy (CTC) of DTI/SEC registration and Articles of Incorporation; 3. Certified True Copy (CTC) of valid business/Mayor’s permit; 4. Duly notarized list with a maximum of three (3) of authorized personnel to transact with DOLE; 5. Payment of required fees of Php 8,000.00 for accreditation registration and Php 2,000.00 for DOLE pass ID. The three (3) authorized personnel listed above per Sec. 4(d) of D.O. 221-A shall submit the following: Photocopy of company ID; Photocopy of one (1) government-issued I Updated contact information; NBI Clearance; Police Clearance; and . 2x2 ID picture. Further, Employer's (company) Authorized Representative shall submit the following: 1, Photocopy of company ID; 2. Photocopy of one (1) government-issued ID; 3.
