3. DESCRIPTION OF DEFICIENCY: DURING A RECENT RIMFIRE INSPECTION (SEQUENCE NUMBER AX20MEM215AXJ31-001) OF A


COMBINING TRANSMISSION A SIGNIFICANT FINDING WAS DISCOVERED. THE INSPECTION REVEALED THAT A BEARING BALL
ANNULAR, P/N-145DS522-2, S/N-ZV526, THAT IS LOCATED WITHIN THE COMBINING TRANSMISSION CARTRIDGE FAN DRIVE WAS IN
AN ADVANCED STAGE OF FAILURE. UPON REMOVAL OF THE BEARING, ITS GENERAL OUTER CONDITION PROVIDED MODERATE
RIVET WEAR. THE BEARING WAS LIGHTLY COATED WITH A LUBRICATION FILM. THE MANUAL SPIN TEST OF THE BEARING WAS
ROUGH TO THE TOUCH AND PRODUCED A LOUD GRINDING SOUND. FURTHER INSPECTION OF THE BEARING REVEALED HEAVY
SPALLING, HEAVY BEARING ROLLING ELEMENT METAL LOSS AND HARD PARTICLE INDENTATIONS THROUGHOUT THE ENTIRE
SET OF ROLLING ELEMENTS. THE BEARING CAGE ROLLING ELEMENT RETAINER POCKETS WERE HEAVILY WORN, DEFORMED
AND ELONGATED. ALSO DISCOVERED WAS HARD PARTICLE INDENTATIONS ON BEARING BALL ANNULAR, P/N 145DS522-2, S/N
ZV536, WHICH IS ALSO LOCATED WITHIN THE COMBINING TRANSMISSION CARTRIDGE FAN DRIVE AND IN CLOSE PROXIMITY TO
THE FAILED BEARING.
THE BEARING BALL ANNULAR IS NOT A MAINTENANCE CONSOLIDATED DATA SYSTEM (MCDS) MANAGED COMPONENT. THE
BEARING TIME SINCE NEW IS UNKNOWN. THE COMBINING TRANSMISSION, P/N-145D5300-20, S/N-2011A1118, WAS RECEIVED AT
THE CORPUS CHRISTI ARMY DEPOT (CCAD) PRE-SHOP ANALYSIS DRIVE TRAIN DISASSEMBLY SHOP AREA WITH A DEPARTMENT
OF DEFENSE FORM 1577-2 (UNSERVICEABLE / REPAIRABLE TAG). THE REASON FOR REPAIR WAS DURING A SERVICEABILITY
CHECK FOUND CHIPS IN FILTER . THE COMBINING TRANSMISSION DEPARTMENT OF THE ARMY (DA) FORM 2410 COPY 1
REMOVAL REPORT BLOCK 12 WAS ANNOTATED WITH A FAILURE CODE 374 (INTERNAL FAILURE). THE COMBINING TRANSMISSION
REMOVAL REPORT ALSO SHOWS THE TRANSMISSION HAS A TIME SINCE NEW (TSN): 925 HOURS, TIME SINCE LAST INSTALLATION
(TSLI): 925 HOURS, OVERHAULS (O/H): 0, TIME SINCE OVER HAUL (TSO): 0 HOURS. THE COMBINING TRANSMISSION DA FOR
2408-20 OIL ANALYSIS LOG WAS NOT RECEIVED WITH THE TRANSMISSION. OIL CHEMICAL ANALYSIS AT CCAD WAS NOT
SUBMITTED DUE TO AN INSUFFICIENT OIL COLLECTION. THE COMBINING TRANSMISSION WAS REMOVED FROM AN ARMY
CH-47F CARGO HELICOPTER, S/N-0808057, NSN-1520014575179, TSN: 1565 HOURS.
