FROM: RIG T-34 T.P HANS G KUNDY MECH PERVAIZ IQBAL, HSE RIZWAN RAHEEM KHAN.

REF: SLIGHTLY PRESSURE LOSS OF NITROGEN IN ACCUMULATOR UNIT.

DESCRIPTION:
WE OBSERVED THAT ACCUMULATOR UNIT WAS NOT HOLDING THE PRESSURE BETWEEN BOP
TESTS. TEST PERIOD IS CONSIST OF 14 DAYS. OBSERVED PRESSURE LOST WAS 100 TO 500
PSI ON MOST OF THE BOTTLES.

IN CASE OF ELECTRIC AND AIR SUPPLY FAILURE WE CANNOT OPERATE ACCUMULATOR UNIT IF
ACCUMULATOR BLADDER HAVE NO PRESSURE.

CAUSES:

 INSIDE CORROSION OF THE NITROGEN BOTTLES IS THE MOST LIKELY CAUSE. BECAUSE
THOSE ARE NOT STANDARDISED.

 ALSO PLASTIC CAPS IS NOT AVAILABLE ON COMING NITROGEN BOTTLES. DURING THE
TRANSPORTATION FROM KARACHI TO RIG SITE GOT A DUST AND SAND PARTICLE INSIDE.

WHAT WENT WRONG:


WHILE CHARGING THE ACCUMULATOR BOTTLES. CORROSION AND DUST PARTICLE WERE
SETTLING ON THE ACCUMULATOR VALVE CORE. AND CAUSE A PRESSURE LEAK.

CORRECTIVE ACTION:
WE INSTALLED A FILTER NOZZLE FROM ACETYLENE REGULATOR ON CHARGING & GAUGING ASSY
IDENT # 024469. SYSTEM IS FILTERING ALL CORROSIVE AND SAND PARTICLE FROM THE
CHARGING GAS TO AVOID THE PRESSURE LEAKAGE.

AFTER THIS ACTION WE GET A POSITIVE RESULT. PRESSURE LEAKAGE IS NOT OCCURRED FROM
26 APRIL T0 12 MAY 2003.
