Aliaa Ahmed,32 years old, G4P3 +0, married for

S
12 years. LMP 18/9/2020 GA 34 and EDD is
25/6/2021. She has fresh red vaginal spotting for
5 days. And lower abdominal stabbing pain.
There is no headache, blurred of vision,
excessive vomiting, DFMC, epigastric pain, fluid
dash or lower limp edema.

She is vitally stable, HB was 65 b/min, regular,

average volume, equal on both sides, vessel
walls not palpable with no special character.
Temperature: 37.2 C BP:130/95
Fundal level at the level of 34 weeks matching
with LMP.
CBC… Hb was 9 mg/dl, platelets count 200.000
UV reveal retro-placental hemorrhage.
differential diagnosis for placenta abruption.

A
1-placenta brevis.
2-gynacological causes of bleeding.
3-systemic causes of bleeding.

UV is the cold stander.


fetal assessment must be done.
Liver function, kidney function, coagulation
profile and RH blood group must be done
Follow up and motoring.

P
Steroids until delivery.
natural progesterone.
If any of complication started, labor pain or any
signs of fetal distress termination must be
indicated.
