INTRODUCTION

Community Health Nursing 2 (CHN 2) is a specialized field in nursing that aims to


enhance the health of people in the communities.It involves interacting with the
members of the community. Specifically it involves preventive care,health education,
and collaboration with local organizations to promote well-being and address health
inequalities.

Community assessment is a comprehensive process that involves examining various


aspects of a particular community such as its demographics,resources and health
needs. This assessment is necessary to identify the community's advantages and
limitations, with this it gives us the proper interventions and strategies to improve the
community

Community Diagnosis involves identifying the specific health needs and problems
within the community through a systematic assessment. This process includes
gathering information on the specific community in order to understand the community
problem, health issues and their underlying causes. The findings or the information
collected is used to create interventions and actions that target the identified health
issues,aiming to enhance the general well-being of the community.

This task will assist barangay officials in establishing guidelines to tackle the recognized
health issues and enhance the overall health conditions of the community. For the
residents, it will provide awareness and valuable knowledge about their present health
status and prevailing circumstances.
OBJECTIVES

General Objectives:

The goal of this study is to provide an overview of the health conditions in Barangay
Inosluban, situated in the city of Lipa, by conducting a thorough evaluation and analysis
of the community's health profile.

Specific Objectives:

This Community Assessment and Diagnosis intends to achieve the following specific
objectives:

•To identify health issues that requires highest priority and need to be treated with
utmost urgency
• To identify the morbidity and mortality cases in the barangay.
• Develop a comprehensive care plan for each identified issue within the community.
• To further improve the community
HISTORY OF THE BARANGAY

Before becoming a formidable barangay, Inosluban was home to just a handful of

people who mainly farmed various crops such as rice, coffee, corn, sugarcane, pepper,

and more. As time passed, more folks found the place, and since the land was fertile,

business people got involved in its development, turning it into a thriving community.

Barangay Inosluban is considered to be one of the biggest barangay out of all 72

barangays in Lipa City,Batangas. Among all the existing subdivisions in Inosloban that

are highly patronized and well-established, St joseph is known as the most popular and

oldest subdivision standing in the area. Moreover, the first semiconductor industry in

Lipa takes place in Inosluban which is PKI(Philippines Kyohritsu Inc..). For the history of

the barangay it was stated by the elders that it comes from a tragedy that happened to

one of the natives named “Ines”. She was robbed and the people shouted the phrase

“Si Ines,nilooban!”. The phrase later became “Inosluban” which is the name used now.
BARANGAY TOPOGRAPHY

Brgy. Inosluban is situated in the northern part of Lipa City, atop the highlands of
Batangas Province. It is bordered by Barangay Bugtong na Pulo to the north, Barangay
Lumbang to the east, Barangay Pusil to the west, and Barangay Marawoy to the south.
Inosluban is conveniently close to the popular Sm Lipa, only a 10-minute drive away,
and approximately 1 kilometer from the Lipa City Hall. There's also an expressway
nearby that leads to Levitown, where you can find a Public Market and a small night
market. This expressway also connects to Fiesta Mall and the Floral Memorial Garden
cemetery.

Old Barangay Organizational Chart

In the previous organizational chart of Brgy. Inosluban, Hon. Miguel L. Olgado served as
the Barangay Captain, leading a team of seven Kagawad members: Elena B. Aguilera,
Merlita P. Garcia, Felix A. Magcawas, Virgilio M. Olan, Omar M. Cabrera, Prescila M.
Manalo, and Marcos V. Magtibay.

For the Sangguniang Kabataan (SK), Roldan M. Mitra held the position of Chairperson,
supported by seven SK Kagawad members: Angelika Mae Castillo, Jasper B. De
Torres, Mary Rose M. Aberia, Maria Elysa Daria C. Llanes, Emiel Bernadeth S. Abello,
Karl Anthony A. Pacaco, and Kenneth M. Briagas. Additionally, Justine Gaylle V. Lit
served as the SK Secretary, while Debbie Vanessa S. Lasac acted as the SK Treasurer.
Recent Barangay Organizational Chart
Following the recent election, Hon. Miguel Olgado retained his position as Barangay
Captain of Inosluban. Assisting him are seven barangay councilors: Hon. Merlita Garcia,
Hon. Elena Aguilera, Hon. Felix Magcawas, Hon. Jericho Dumlao, Hon. Omar Cabrera,
and Nestor Balita. Together, they handle tasks such as creating local laws, representing
the community, joining committees, and providing public services.

In addition, Hon. Ashlie Nicole Gonzales was elected as the SK Chairman. She leads a
team of seven SK Councilors: Faye Abueg, Gemma Marquez, Jean Michelle Maoirat,
Lawrence Macahia, Gian Carlo Barairo, and Mylene Joycee Calise. Their focus is on
youth-related matters within the barangay.

With that being stated they are committed to enhancing the welfare of Barangay
Inosluban. Through collaborative efforts, they aim to enact positive changes, address
community needs, and foster a better living environment for all residents.
BARANGAY HEALTH CENTER ORGANIZATIONAL CHART
The Health Center of Barangay Inosluban is staffed by several workers and officials. Dr.
Madonna N. Suan serves as the medical officer, responsible for organizing health
education initiatives and overseeing the community's medical documentation.Nurse
Amy L. Cuenca and Nurse Elsa K. Gutierrez work under her. Additionally, Midwife Mary
Ann K. Manalo, whom we interviewed, provides us with the data for our barangay.. Next
is the Barangay Health Workers(BHW) which consists of Marilyn B. Latina,Juliet
M.Libao,Evelyn M. Macahia,Lolita K Mercado,Hermina G. Jaralve,Elena C
Llanes,Rosemarie K Canas,Rowerna M, Mercado,Nancy S Lasac,Esperanza S
Olan,Janine R Cuenca and lastly is Chink M. Marquez teaches people about health,
and helps connect residents with health care places. They play a big part in making the
community healthier. And Lastly is the Barangay Nutrition Scholar which consists of
Concepion O Rosales,Emelina C Rogado and Liezel B Latayan. They assess and
educate the community on proper nutrition.

All of them function as the barangay's frontline responders, addressing residents'


immediate health needs and promoting overall well-being while preventing diseases
within the community
BARANGAY SPOT MAP

Barangay Inosluban in the highlands of Batangas Province spans 642.858 hectares.


Residential areas cover 75%, Agricultural land is 2%, Commercial zones make up 23%,
and there is no idle land. The barangay is linked to neighboring barangays in Lipa
through an expressway, offering convenience but causing traffic congestion in specific
areas due to its connection to the main road.

Barangay Inosluban's population consists of diverse age groups: 952 aged 0-5, 2,901
aged 6-17, 6,458 aged 18-59, 1,003 aged 60-80, and 55 individuals aged 81 and above.
There are 2,302 households, 131 disabled persons, 1,058 senior citizens, and a total of
2,872 families in the barangay.
MULTIFACTORIAL PHENOMENA

A. POLITICAL FACTORS

The political atmosphere in the barangay remains peaceful and devoid of


controversy. While differing opinions and party affiliations surface during
elections, there exists a strong culture of respect for each individual's voting
choices. The Barangay Peace and Order Committee, along with the Barangay
Police Force and Lupong Tagapamayapa, work in conjunction with the Barangay
Officials to ensure the maintenance of peace. Currently, upholding peace and
order does not impede the barangay's progress.The execution of plans will be
the exclusive responsibility of the barangay, led by Punong Barangay Miguel L.
Olgado, with input from the Sangguniang Barangay members and other
functionaries, fostering a closer relationship between the youth and adults
through collaborative efforts. The community's objectives revolve around
enhancing infrastructure, agriculture, and livelihood projects to bolster overall
development.There is also an active participation on the part of the community to
further ensure the continued progress of the Barangay.

B. SOCIO-CULTURAL FACTORS

As per the barangay officials, the population composition of Barangay Inosluban


comprises 88 percent Roman Catholic, 10 percent Iglesia ni Cristo, and 2
percent Baptist. The barangay observes its annual feast on the 1st of May, with
community members actively preparing various dishes for anticipated visitors.
Festivities include organized programs by barangay officials, featuring small
concerts, games, and vibrant decorations. Moreover, street performances by
bands and carnival attractions add to the celebratory atmosphere.
C. ECONOMIC FACTORS

Inosluban, one of Lipa's largest barangays, offers abundant job opportunities.


Numerous subdivisions and the PKI contribute to employment, fueled by
increased demand and expansion. The area boasts an old factory(PKI),
alongside amenities like coffee shops, restaurants, a gym, and automotive
services, providing diverse job options. A nearby public school benefits street
vendors, adding to income sources. According to barangay officials, most
families fall into the average-income bracket, relying on farming, business, and
employment. The barangay facilitates job information dissemination through
bulletin boards and public announcements.

According to information from barangay officials, the primary communication


networks used in the area are Globe, PLDT, and Smart. In terms of
transportation, community members commonly use tricycles, jeepneys,
motorcycles, buses, and private vehicles, making travel within or outside the
barangay convenient and straightforward.

D. ENVIRONMENTAL FACTORS’

1. Physical/Geographical/Topographical Characteristics of the Community

Barangay Inosluban is situated in the highlands of Batangas Province. According


to barangay records, the land area spans 642.858 hectares, with the following
land use distribution: Residential areas cover 75%, Agricultural land comprises
2%, Commercial zones make up 23%, and there is no idle land. The barangay is
well-connected with an expressway linking it to other barangays in Lipa,
providing convenience but contributing to traffic in certain parts due to its
connection to the main road. The road widening initiative has not only improved
transportation but has also enhanced the overall organization of the barangay.

The climate in Inosluban is akin to that of the city and nearby towns and
provinces, falling under the first type of tropical climate with two distinct seasons
throughout the year. Typhoons are common during the rainy season,
occasionally causing damage to agricultural products and crops. This results in
environmental challenges for the community.

2.Water and electric supply

As per the barangay authorities, every member of the community has access to
safe and drinkable water, with the water source being the Metro Lipa Water
District. Initially, when we first arrived, there were occasional issues with water
pressure, particularly on weekends, but these concerns were addressed over
time. Regarding the electricity supply in Barangay Inosluban, it is provided by
BATELEC II, and it has proven to be reliable, even in the face of challenges such
as strong typhoons.

3. Waste Disposal

Every week, a garbage collector visits the barangay to ensure that every
household's waste is collected, promoting cleanliness and preventing potential
health issues for residents. The barangay adheres to strict waste disposal
practices, but there is a common issue where residents often neglect to
segregate their garbage properly. Segregation involves sorting waste types to
facilitate recycling and appropriate disposal. During our interview with a councilor,
it was mentioned that there are occasional delays in garbage collection due to
various reasons. One factor is the inoperable garbage truck, while another
challenge stems from a conflict regarding the location of the main garbage
disposal site. These issues contribute to delays in garbage collection, potentially
resulting in the accumulation of waste within the community.

4. Air,Water and Land pollution

Due to the delayed garbage collection, some residents resort to burning


accumulated waste, often using dried leaves, leading to air pollution. This is
particularly concerning in the afternoon when many children are playing outside.
Inhaling the smoke from burning waste may result in respiratory problems,
discomfort, headaches, nausea, and could trigger asthma among residents.
According to the midwife, or at least her assumption, the nearby PKI factory
might contribute to the increased health issues in our barangay. Additionally,
certain areas within the barangay have turned into unofficial dumping sites,
causing drainage systems to become clogged.

E. PHILIPPINE HEALTH CARE DELIVERY SYSTEM

The healthcare delivery system encompasses a structured network of


institutions, healthcare providers, and resources dedicated to offering medical
care and services within a specific area or nation. In the Philippines, this system
is intricate, combining public and private entities, functioning within a
decentralized framework that spans from community-based initiatives to
advanced tertiary healthcare facilities. Its primary objective is to provide medical
services and foster health throughout the country.
The Barangay Health Center (BHC) is conveniently situated near the Basketball
Court within the barangay hall, ensuring easy access for all residents.
Operational from Monday to Friday, 8am to 5pm, it caters to various health
needs. Services offered include first aid, maternal and child healthcare,
management of communicable diseases, and addressing prevalent health
concerns within the community. They also stated they are more on prevention
and referral systems.

The center provides prenatal education and checkups, offering crucial


supplements to expectant mothers. Barangay health workers aid in supporting
mothers and children and administer vaccines for infants. Additionally, adults
receive anti-pneumonia and anti-flu vaccinations.Operation Timbang (OPT) is a
service aimed at tracking malnutrition among children aged 0-71 months within
the barangay and was implemented by the BHW according to their assigned
purok.Additionally, the BHC provides family planning programs, offering
contraception methods like pills and condoms. These health workers act as
crucial frontline figures, delivering fundamental health services that uphold the
primary healthcare approach, emphasizing accessible and acceptable health
services within the barangay.

In 2022, animal bites led to morbidity with 150 cases in Barangay Inosluban, followed by
upper respiratory tract infections (36 cases) and skin rashes (37 cases). In 2023, animal
bites remained the top concern but decreased to 77 cases, while URTI slightly
increased to 38 cases. Influenza emerged as the new 3rd, reporting 5 cases.

Animal bites can cause various health problems, leading to sickness in affected
individuals. These bites often come from animals like dogs, cats, and wildlife such as
bats or snakes, and they can bring a risk of infection due to the germs in the animal's
mouth or on its skin. s. If not treated properly, complications like cellulitis, abscesses,
and systemic infections can occur. There's also a serious risk of rabies,requiring quick
medical attention and preventive measures. Allergic reactions, emotional impacts like
fear or anxiety, and the strain on healthcare systems are also important parts of the
problems caused by animal bites. Preventing these issues involves proper wound care,
responsible pet ownership, and public health efforts.

The mortality data for the period spanning January to September 2023 reveals
significant insights into the leading causes of death within the barangay. Topping the list
is senility, accounting for the highest number of reported cases at 5. Following closely is
myocardial infarction with 4 cases, and pneumonia takes the third spot with 3 cases.
Other notable contributors to mortality include hypertension, diabetes, acute respiratory
failure, lung cancer, renal disease, multi-organ failure, and colon cancer, each reported
with varying frequencies. This ranking provides a snapshot of the prevailing health
concerns and can guide health authorities and practitioners in directing resources and
interventions towards addressing these specific health issues within the community
