AFFIDAVIT OF LOSS

I, ORLANDO A. BING, of legal age, Filipino and a resident of Davao

City, Philippines, after having been duly sworn to in accordance with law, do hereby
depose and say that:

That I am the holder of the following identification cards and documents,

which I placed inside my wallet, to wit:

a. City Government ID
b. LTO License
c. PhilHealth

That on May 3, 2020 my wallet containing my above-enumerated documents

and identification cards was lost and believed to be fallen somewhere while I was
riding a motorcycle;

That despite diligent efforts exerted to locate my above-mentioned wallet

containing my above-enumerated documents, the same could no longer be found
and the above-enumerated documents are now deemed lost and beyond recovery;

I am executing this affidavit to attest to the truth of the foregoing facts and to
inform the proper authorities concerned of the facts aforementioned for whatever
legal purpose/s this may serve.
