Introducing the community project to the Community Health Center together with the
November 11, 2023 Barangay Captain, Barangay Kagawads, and Barangay Healthcare Workers.
(Saturday) Brainstorming and planning on the community project about prevention of teenage
pregnancy and choosing of place for seminar (Barangay Hall of Brgy. San Jose)

November 14, 2023 Research about the teenage pregnancy in the community to narrow down ideas.
(Tuesday) Creation of brochure for participants

Developing of proposal for action.


November 16, 2023 Collaborate with Barangay Health Center
(Thursday) Continuation of research create the action plan for the service.
Recording all information and developments.

November 18, 2023 Carry out each step of the service as action plan.
(Saturday) Record all the process and developments. Photos of the action must be included.

Evaluations
POST- ACTIVITY EVALUATION
1. The research that has been conducted on Brgy. San Jose,
Navotas city will provide insights and factors that contributes
to teenage pregnancy. This will help them provide a better and
effective program on prevention of teenage pregnancy.

2. With the help of research, the community of Brgy. San Jose,


Navotas City will provide an effective program to lower down the
number of female teenagers who gets pregnant at the early
age.
POST - ACTIVITY EVALUATION
3. The teenagers is able to access to reproductive health services
in their Barangay and have an adequate knowledge through
counselling, family planning and the proper usage of
contraceptives.

4. The participants will have a positive feedback on the program


and is able to share their own perspectives on the effectives of
prevention activities.

NARRATIVE REPORT
The program is entitled “Children having Children” A Community Program on the
role of the Brgy. San Jose, Navotas City in preventing Teenage Pregnancy. One of the
participants is isabela Gallathea. She is 18 years old and lives in Brgy. San Jose, Navotas
City.
In this program, there will be 50 participants. 25 male and 25 female teenagers. The
people who are also included in here are the Barangay Captain, Barangay Kagawads, SK
Chairwoman, SK Kagawads, Tanods, Barangay Health Workers, the Guest Speaker, the
Emcee, and the Dance Troupe. All in all, there will be 100 person for this program. Most of
the materials which are contraceptives are all available in Health Center. The other
materials such as the microphone, chairs, tables, and speakers are all available in the
Barangay Hall. In this program, the needed budget is 6,000 pesos. This budget is for the
tarpaulin, brochures, talent fee, and the token for the guest speaker. We will ask for
sponsorship to Natural Spring Company for water bottles or refreshments.
For the program to be successful, we will collaborate first with the Barangay San
Jose about our plan and start making research about the teenage pregnancy in their
community. After that, we will continue in conducting the research and formulate a plan.
NARRATIVE REPORT
After formulating a plan, we will asks for a sponsorhip to Natural Spring Company
for water bottles or refreshments. After that, on the day of the program, the guest speaker
will focus on the level of teenagers who gets pregnant at the age in Brgy. San Jose,
Navotas City. The guest speaker will also discuss about the prevention of teenage
pregnancy through by using of contraceptives, decision-making skills, family planning,
and communication with their partners. After the program, the Baranggay health workers
will provide a counselling to those who want to have a counselling and will provide
contraceptives to everyone that wants to have. The participants will also have a time
wherein they can asks questions regarding the topic that has been discussed. After that
we will conduct an evaluation about the program.
After that day, we will identify if our objectives and goals was met. We will check if
the rate of teenagers in Barangay San Jose Navotas City gradually lowers down or it
remains stagnant. With this program, it will help the community of San Jose to provide a
better plan to lessen the number of teenagers who gets pregnant at the early age.
