Part 3: Prioritise 3 actions with justification for the selection
Suggested word counts
Moral and financial arguments for all actions: 300 to 350 words
For EACH action:
Specific legal arguments: 100 to 150 words
Likelihood AND severity: 75 to 150 words
How effective the action is likely to be in controlling the risk: 100 to 150 words

Moral and financial arguments for ALL actions

Moral, general legal and financial It's the employer's moral responsibility to provide a safe system of work to the workers. The peoples
arguments work in an organization for feed their families, and this is not acceptable that he/she killed or injured
by the job. It is a basic responsibility of a firm to implement positive safety culture because in the
absence of the safety culture worker will face lifetime injury, psychological stress and even a chance
of loss of job due to accident, not only the worker will suffer but also his family suffers a financial
crisis. Workers and society can’t accept putting their lives at risk, so the employer has to provide a
safe workplace for working without hazards.
In the case of if the worker is injured due to irresponsibility of the organization, the firm will face the
direct cost in the form of worker sick pays, immediate first aid treatment, cost of an accident, and in
the absence of the worker hiring of the new worker. Accident and ill health have a major effect on the
financial assists of company, replacement of the damaged tools and equipment’s, vehicle, etc and
the workers will not perform their regular job with focus due to investigation and will not feel safe at
the workplace. There will a major time loss to prepare reports for enforcement agencies and fines
from court during legal proceedings, and will face loss of company’s credibility, reduction in sales,
etc. In an HSE search, it was demonstrated that uninsured cost is 8 to 36% higher than insured cost.
The legal requirements of an employer are to provide a safe system of work to the workers, safety
equipment, safe machinery should be provided under strict supervision to prevent any hazard. If any
hazard will occur then enforcement agencies of the country will take action against the company, fine
from the court, hiring of lawyers, and even a major hazard will occur then enforcement agencies will
cancel the license of the company. So, it's the company's responsibility to follow the international
labor organization convention C155 and Recommendation R164 which tells us that employers

Learner number: Learner name: Ahsan Ali Page 16 of 21

should ensure that machines, equipment, and workplace under their control are as safe as possible
and without health risk.

Justification for action 1

Barrication of cross scaffold tubes should be provided at the outer edges of the roof.
Action
(Hazard Category: Work at Height)
Specific legal arguments According to ILO Convention C155 - Occupational Safety and Health (Article 16) clearly state
that employers should be provided a safe workplace, machinery, process under their control is
without any risk. ILO Convention C167-Safety and Health in Construction (Article 18) stating that
necessary preventive measures should be taken against the falling of workers, equipment, and
objects from the roof. International labor office training programmed theme summary “Work at
Height” states that on roof work not work near the edge, the simple barrier should be provided
consisting of cross scaffold tubes from avoiding any risk.
Pakistan OSH Act 2018 states that the government of Pakistan should implement the Convention
and Recommendation given by the international labor organization (ILO).
Consideration of likelihood AND the The likelihood of the hazard is very likely because the worker working on the rooftop near the
severity edges, no safety harness worn by the worker which increases the falling chances of the worker
from the rooftop.
When signifying the severity, I concern about the following table
 Negligible: No injury will be happened
 Minor: Minor abrasions, bruises, cuts, first-aid type injury
 Serious: Non-fatal injury, hospital treatment, disability may occur
 Fatal: Permanent disability, Major property damage, equipment, and machine loss
 Catastrophic: Irretrievable property loss and production, Death will happen
The severity of the hazard I will be rate from the table fatal to catastrophic because in the case of
falling the worker from the 3rd floor top requires immediate hospital treatment and should lead to be
fractures, mental illness, elbow fractures, internal organs, or limb injuries, depression, backbone,
and even death.
How effective the action is likely to be in
Availability of the scaffold cross tubes at the roof edges will efficiently reduce the chance of falling
controlling the risk. This should include:
from the roof and will ensure the worker works safely without any fear.

 the intended impact of the action; I have given the timescale for this action to accomplish was 6 weeks because the finance manager
 justification for the timescale that you will first buy the scaffold tubes and a separate budget will be owed for it, the time after buying to
indicated in your risk assessment; assemble as cross scaffold barrier and after it should be placed at edges of the roof. I am hopeful
and that the timescale given for this action is manageable.
 whether you think the action will fully
control the risk. This control measure can’t fully control the risk associated with the rooftop, safety nets, airbags,
and safety harness should be provided that may fully control the risk.

Justification for action 2

Introduce residual current devices to save human beings from sudden electric shock.
Action
Hazard Category (Electricity)
Specific legal arguments The ILO Convention C155 and Recommendation R164 require from employees that the
workplace and the processes under their control are safe for the workers. The ILO convention C167
- Safety and Health in Construction (Article 26) states that appropriate measures should be
taken before and during construction to determine that any live cable or electrical equipment at the
site or the site exposes a risk to workers. In the ILO encyclopedia, general hazards in chapter
40 state that residual current devices should be used from preventing the live cables touch at the
site because they can trip by currents as low as 30 mA with potentials as low as 30 ms. Pakistan
OSH Act 2018 states that the government of Pakistan should enforce the workers and implement
the Convention and Recommendation given by the international labor organization (ILO).
Consideration of likelihood AND severity The likelihood of the associated hazard is frequent that live cables are hinged everywhere at a
construction site, no safety shoes were worn by the workers, no residual devices were installed that
will high the risk of the hazard occurring.

The severity of this hazard is fatal to catastrophic because the chance of death will be 80 to 90%.
Due to the live cables hinged direct or indirect burn, muscle cramp, cardiac arrest, Tissue burns,
painful shock, and even death will be occurred due to sudden interaction with the cables.
How effective the action is likely to be in
After installation of residual current devices there will be minimum chances of a major hazard
controlling the risk. This should include:
occurring due to living cables touch because they will be tripped at low current as low as 30 mA.
 the intended impact of the action; The timescale given for this action is 3 Months because the HSE Officer first contact with finance
 justification for the timescale that you manager for buying these devices, and after buying these current devices installation at the
indicated in your risk assessment; 22763.6 m2 area consumes a lot of time so that 3 months are sufficient to complete this action.
and
 whether you think the action will fully This control measure can’t fully control the risk associated with the live cables but reduce it to
control the risk. approximately 90 percent, but if earthing, double insulation of wires and low voltage system will
apply then hopefully completely control the risk.

Justification for action 3

Mechanical ventilation or exhaust fans should provide to remove humidity and balance the oxygen
Action
content. Hazard Category (Confined Space)
Specific legal arguments The ILO convention C164 and Recommendation R164 describes that employees should ensure
that processes running under their control are safe for the workers. The ILO convention C152 -
Occupational Safety and Health (Dock Work) Article 32 describes that take appropriate
measures to prevent accidents or harm to health when an employee is asked to enter a confined
space where toxic or harmful substances have been found or there may be no oxygen etc. The ILO
Recommendation R097 - Protection of Workers' Health Recommendation states that avoiding
exposure by removing dust, smoke, gas, fibers, fog, or fountains at or near the site of origin by
using mechanical exhaust ventilation systems and other suitable methods. Pakistan OSH Act 2018
states that the government of Pakistan should enforce the workers and implement the Convention
and Recommendation given by the international labor organization (ILO).
Consideration of likelihood AND severity The likelihood of this hazard is very likely because at any time a heavy amount of water will come
out then there will be a loss of life. After all, the worker was gone in the pipeline without the
respiratory system that will enhance the chances of the hazard occurring.
When signifying the severity, I concern about the following table
 Negligible: No injury will be happened
 Minor: Minor abrasions, bruises, cuts, first-aid type injury

Learner number: Learner name: Ahsan Ali Page 19 of 21

 Serious: Non-fatal injury, hospital treatment, disability may occur
 Fatal: Permanent disability, Major property damage, equipment, and machine loss
 Catastrophic: Irretrievable property loss and production, Death will happen
The severity of this hazard is fatal to catastrophic because without the respiratory system you will
not live an hour due to suffocation, drowning fear, etc. Entrants will be affected by the lack of
oxygen and will not be able to move freely through the pipes. They will face the explosion, loss of
consciousness, drowning, high temperature, and contamination which can lead to death.
How effective the action is likely to be in
By implementing this control measure, it will be ensuring that the level of suffocation will be reduced
controlling the risk. This should include:
to a major level. Workers who work in a confined space will feel relax during their work.
 the intended impact of the action; The timescale given for this action is 2 Months because the HSE Officer firstly contact the finance
 justification for the timescale that you department for hiring an HVAC engineer for the installation of mechanical ventilation on a contract
indicated in your risk assessment; basis for three months. Installation and running of mechanical ventilation and exhaust fans system
and are enough to complete in 3 months.
 whether you think the action will fully
control the risk. After implementing this action according to my thinking the possibility of the hazard will overcome but
not completely remove, but if you also implement permit to work system, enforce the workers to wear
scuba and other PPEs for saving their lives.
Part 4: Review, communicate and check
Suggested word counts for each section:
 Planned review date or period and reasoning for this: 50 - 100 words
 How the risk assessment findings will be communicated and who needs to know the information: 100 - 150 words
 Follow up on the risk assessment: 100 - 150 words.

Planned review date/period with It's company policy that they review risk assessments after 12 months. So, I am setting the date of
reasoning reviewing the risk assessment as 30 July 2022. However, it is also be reviewed that before the date
of risk assessment is there will be any change occur like
 Number of workers, working time, company policy will be change
 New machinery or any new equipment will be installed
 Working standards, Health and Safety Rules will be change
How the risk assessment findings will be I will call a meeting in which the Finance Manager, HSE Officer, Supervisors, Procurement
communicated AND who you need to tell department will come, and I tell them to implement the necessary actions describes in the risk
assessment report. I will also provide a summary of the risk assessment report and findings, actions
for the site. The findings of the risk assessment report will be included in the upcoming toolbox talk

Learner number: Learner name: Ahsan Ali Page 20 of 21

in which I will advise the employees, supervisors, etc. to follow the actions that are to be taken. I will
also tell them that provide training to the newly hired workers, that they will not face any hazard occur
before their presence. A summary of the risk assessment will also be posted on the company intranet
that all workers will have access to.
How you will follow up on the risk I will set a diary reminder for 10 days before the long-term actions to accomplish and set a monthly
assessment to check that the actions reminder for visiting the site that the action is implemented or not. I will check the progress on daily
have been carried out basis, and if I found any action that is not completed I will find the reason behind it and meet the
responsible person to accomplish that action. If a financial problem will occur then I will meet
directly with the Finance Director for completing the actions that are pending due to financial
reasons. And if some actions are not completed due to time then I will allocate extra time to the
related person for the completion of the target.
