AFFIDAVIT OF GUARDIANSHIP

I, MARIO E. NARANJO, of legal age, Filipino Citizen, and resident of Brgy. Binday,
San Fabian, Pangasinan, after having been sworn to in accordance with law, hereby deposes and
states that-

1. I am the brother of the late ANABELLE EMPERADOR GENERAO and the brother-in-
law of the late NOEL SALVATERA GENERAO;

2. My late sister and brother-in-law, has left in my custody her daughter, JASMINE
NARANJO GENERAO;

3. My niece JASMINE NARANJO GENERAO, is now under my care and living with me
being the nearest kin to the family;

4. I hereby declare myself as the de facto guardian of my niece, JASMINE NARANJO

GENERAO;

5. I am executing this affidavit to attest to the truth of the foregoing facts which I have
personal knowledge of and also for the application of The Department of Science and
Technology scholarship of my niece, and for whatever legal purpose this may serve.
