d. Has orderly, accurate and updated teachers’/ learners’ portfolio such as class record, item
banks and analysis

e. Well-structured classroom

2. Had been a Demonstration Teacher 10 %

3. Had been a trainer / speaker in an in-service training 10

a. National level

4. Conducted an action research for the improvement of instruction 10 %

(every approved action research)

5. Developed and utilized instructional materials that have contributed to the 10%

improvement of delivery of instruction

(teaching devices, apparatuses, modules, activity cards)

6. Had been a recipient of an employee award/recognition in the field of education 10%
