DOCUMENTS REQUIRED (scans):
• Birth Certificates (Tazkiras) for all family members.
• Family Booklet and/or Civil Registry Extract.
• Marriage Certificate (if applicable/available)
• Divorce certificate (if applicable/available)
• Custody/court documents for your children (if applicable/available)
• Baptismal certificate (if applicable/available)
• Military booklet (if military service was completed or an exemption
was obtained)
• National identity cards (if available)
• ID from country of residence (if available).
• Complete pages of all valid and/or expired passport(s) within the last
10 years for each person included in the application. If an applicant
does not possess any valid or expired passport then they may
provide a copy of their national identity card (or other official
documentation with a photograph) for identification purposes

Schedule A Background/Declaration Form
 For principal applicant and all dependents 18 years old and above (if
applicable).

 If a question is not applicable to the applicant, write "N/A." Do not use


general words such as "employee", "working", "manager". You must
indicate “unemployed” if the applicant was not working during a certain
period.

 No gaps allowed in questions 7, 8, 9, 10, 11, 12.

 Applicants must account for every month and year of Personal


history/Employment since the age of 18 or for the past 10 years
whichever is most recent. If the applicant is a retiree, question 8 must be
filled back to 10 years prior to retirement.

electronically fillable. Any gaps in the form(s) will result in it being
returned and cause additional delays.

De-Facto Family Members

 Our current priority is you and your immediate family


members;
 A de-facto family member could be: widowed mother, an
unmarried sister etc;
 Definition:
 The migration officer must be satisfied that these persons are
dependent on the family unit in which membership is claimed and
cannot apply as a family member. The dependency may be
emotional or economic and will often be a combination of these
factors. Such persons would normally, but not exclusively reside
with the principal applicant as members of the same household.
 Any fraud associated with a de-facto family member
will not be tolerated.;
 De-facto family members will be assessed on a case by
case basis.
