AFFIDAVIT OF CANCELLATION

I, GEORGE MINDAROS GERVACIO JR., of legal age, Filipino, married,

with postal and residence address at B 7 L12 Opal St., Perpetual Village 7,
Mambog V, Bacoor City, Cavite, after having been duly sworn in accordance
with law, depose and say THAT:

1. I am a registered owner of a business under the trade name DIMPLE

TRANSPORT SERVICES located at B 7 L12 Opal St., Perpetual Village
7, Mambog V, Bacoor City, Cavite;

2. The above-stated business was duly registered at the Department of

Trade and Industry (DTI) on MAY 10, 2022, and was issued a Certificate
of Registration with No.: 3845994;

3. At present, I am causing the cancellation of the said registered business

due to bankruptcy;

4. I am executing this affidavit to attest to the truthfulness of the foregoing

declarations under penalties of perjury to respectfully request the
Department of Trade and Industry (DTI) to cancel the Certificate of
Registration No: 3845994, and for whatever legal purposes this may
serve.
